Folowing my last post : How would you manage this patient ? 76 y.o gentleman with SAVR and CABG( Saphenous vein to RCA ) 10 year ago. Come to our emergency with chest pain and syncope, whith the diagnosis of NSTEMI. Echo showing a LV/Ao (mean gradient of 62 mmHg ,Peak of 96mmHg) , LVEF=40%, anterolateral hypokinesia. -CKD : stage 2 -COPD:mild/smoker This is the angiogram : What is your opinion? Our Heart Team made the decision for PCI (IVUS + IAB) and 1 week later the TAVR procedure with a self-expanding supra annular valve. Our OCT Imaging system enhances procedure success and outcomes, especially in calcified lesions. ✅ The ILUMIEN IV Calcified Lesions Substudy demonstrated OCT-guided PCI improved procedural and long-term patient outcomes, especially with safety compared to angio-guided PCI. ✅ The ECLIPSE trial showed in severely calcified lesions, Abbott’s OCT was associated with significantly lower 1-year event rates for vessel preparation with both orbital atherectomy and conventional balloon angioplasty. ✅ Earlier evidence from the CALIPSO trial presented at EuroPCR 2024 showed OCT-guided PCI was superior to angiography-guided PCI in complex calcified lesions with greater minimal stent area (MSA) & no difference in contrast usage and procedure time.
