#diabetes #surgery #medicine #medicalsciences https://lnkd.in/gV__D4ji People with type 2 diabetes who underwent #bariatric #surgery compared to medical or lifestyle interventions had better long-term outcomes, such as greater diabetes remission and less reliance on medication. Courcoulas et al. pooled the results of four separate randomized clinical trials designed to evaluate the effectiveness of bariatric surgery, compared with combined medical therapy and lifestyle management, in improving glycemic control in participants with T2D and a body mass index of 27–45. Bariatric surgery in patients with T2D proved more effective than other interventions and, in contrast to those interventions, resulted in long-term improvements in glycemic control, high-density lipoprotein cholesterol and triglycerides. Increased rates of diabetes remission and reductions in medication use were observed in surgically treated participants, relative to results obtained for those relying on medical or lifestyle management alone. Although the rates of major adverse events were comparable between the groups, patients undergoing bariatric surgery had a higher risk of nutritional deficiencies, gastrointestinal adverse events and bone fractures at long-term follow-up.

Bariatric surgery may help induce spontaneous ovulation in women with polycystic ovarian syndrome (PCOS). The Lancet recently published the BAMBINI trial results – a multicenter, open-label RCT of 80 adult women (median age 31, 79% White) with a diagnosis of PCOS and a BMI < 35 kg/m2 who were randomized 1:1 to vertical sleeve gastrectomy or behavioral interventions/medical therapy. The primary outcome was the number of ovulatory events over 52 weeks, biochemically confirmed using weekly serum progesterone measurements. Overall, women in the surgical group had 2.5 times more spontaneous ovulations compared to the medical group (incidence rate ratio 2.5, 95% CI 1.5-4.2). There was significant weight reduction in the surgical but not medical therapy group. There were also more adverse events in the surgical group (66.7% with surgery (wound infections/nutritional deficiencies) vs. 30% in the medical group (primarily nutritional deficiencies)), although without long-term sequelae. When i was comparing the long-term costs of GLP-1 medications and bariatric surgery, i was very surprised with what i found out. No question that the treatment of obesity and its related diseases have to be done with multi modality for better long term results. The finds are bellow: • GLP-1 medications (e.g., semaglutide) cost $10,000-$20,000 annually, requiring lifelong use, potentially amounting to $100,000-$400,000 over 10-20 years. Insurance coverage is inconsistent, and discontinuation often leads to weight regain. • Bariatric surgery costs $15,000-$30,000 upfront, with potential follow-up expenses. Surgery often leads to significant, sustained weight loss and health improvements, reducing the need for other treatments. Bariatric surgery is more cost-effective long-term due to its one-time cost and lasting health benefits, while GLP-1 medications involve high, recurring expenses.

Both SR-LRYGB (silastic ring laparoscopic Roux-en-Y gastric bypass) and LSG (laparoscopic sleeve gastrectomy) cut down the need for diabetic and heart medications. SR-LRYGB did even better, reducing the need for diabetes meds by 79% compared to 55% with LSG. 🧘 After surgery, the need for psychiatric medications went up by 133%. 🫀 Use of heart medications dropped by 56% Despite some hesitance around bariatric surgery, it proves to be beneficial. This study confirms that obesity care requires a comprehensive approach, including mental support. Mood continues to play a crucial role in overall wellness and it is important to ensure long-term care that includes mental, nutritional and digestive health. Great insights! It's clear that both bariatric surgery and GLP-1 medications present significant financial barriers for many people. Neither option is truly affordable yet, especially for those without insurance coverage. I hope that, as time goes on, weight loss medications will become more cost-effective and accessible to a broader population. Insurance companies have a crucial role to play in making weight loss solutions – surgical or otherwise – more achievable for those who need them.
