I heard a colleague discussing the "knowledge gap," emphasizing the importance of staying current with literature and advancements in our field. As a wise man once said, "The greatest enemy of knowledge is not ignorance, but the illusion of knowledge. While I recognize the profound truth in the fact that doctors should stay current in their fields, I also believe it's incomplete within the context of our society. Protecting Participants Is Not the Top Priority in Clinical Research. That is the title of a Viewpoint I wrote that just got published in JAMA. The title is of course an accurate statement of the ethical and regulatory principles that apply to clinical research. Unfortunately, the Declaration of Helsinki, widely considered to be the "'cornerstone' document pertaining to medical research ethics," prominently says just the opposite: that the goal of generating new knowledge "can never take precedence over the . . . interests of individual research subjects." As I explain in my piece, this misstatement by the Declaration causes real harm to our efforts to ethically conduct clinical research. On the 60th anniversary of the Declaration this month, it is appropriate to seriously think about correcting this misstatement, so that the Declaration can continue to play a positive role in making clinical research more ethical. 60 years ago the Declaration of Helsinki was adopted. It was the first set of international ethical principles guiding medical research involving human participants and is regarded as the most important document in the history of research ethics. To keep up with the changing medical research landscape and the evolving global ethical challenges, revisions of several revisions to the declaration have been made in the past. Recently, the World Medical Association (WMA) has announced the adoption of the 2024 revision of the Declaration of Helsinki. Our article entitled "Ethical Issues in Implementation Science: A Qualitative Interview Study of Participating Clinicians" is now available. Implementation studies present ethical issues that traditional research ethics frameworks and concepts do not address so we have begun to address this gap by presenting the results of interviews with clinicians working at sites currently participating in an implementation study seeking to improve patient handoffs to the intensive care unit (ICU).
