Midlife women should be advised that brain fog during perimenopause is very common, changes in cognitive performance are typically mild and within the limits of normal, and dementia at midlife is rare. Hormone therapy is currently recommended for maintaining brain health only in women who undergo oophorectomy before the typical age of menopause. Hormone therapy may be helpful in alleviating menopause symptoms that may contribute to cognitive problems during midlife. In younger, naturally menopausal women with minimal menopause symptoms, there is no reliable evidence of cognitive benefit or harm with HT. Further, randomized trial data indicate increased dementia risk with HT use in older postmenopausal women. Modifiable risk factors can optimize midlife health and decrease risk for future dementia. Brain fog, referring to menopause-related subjective cognitive difficulties, is common in midlife women. Longitudinal studies find small but reliable declines in objective memory performance as women transition into perimenopause, and these are not explained by advancing age alone. When memory declines occur, performance levels remain within normal limits for all but a very small number of women. Women’s experience of brain fog extends beyond memory complaints, reflecting the negative effect on a broad range of cognitive abilities. Forty to sixty percent of midlife women report cognitive symptoms such as forgetfulness during the menopause transition. These symptoms manifest as brain fog, which reflects difficulty remembering words, names, anecdotes, and numbers; an inability to focus and concentrate; and distractibility. Some women may report and show signs of attention deficit hyperactivity disorder.
