Next week's webinar: https://lnkd.in/eD3dfwvN The webinar addresses the value and importance of investigating Safety Instrumented Systems (SIS) failures to root cause. Many companies do not understand why this is important because of either a lack of understanding, manpower issues and/or time pressure to restore the SIS. The webinar emphasizes the fact that IEC61511 requires SIS failures to be investigated, therefore, we need to understand how the SIS has failed. For example, did it fail-safe, fail dangerously, was it a random failure or a systematic failure? Many times, SIS failures are either dismissed as being systematic or simply having the faulty equipment replaced without fully understanding why it failed and how. It should also be remembered that the probabilistic calculations performed on Safety Instrumented Functions (SIFs) are based upon random failures. If failures are not properly classified, then this will lead to lower random failures, which in turn can lead to very low and dangerously optimistic estimates on PFDavg or PFH. I realised I didn’t shared, as I usually do, the questions asked after the SafeComp presentations. Since the INCOSE presentation of IEC 63187 paper that start to make a long list of questions that can be worth a detailed answer. Maybe I should start a poll if some of you are interested by answers to some of those questions. Let me know in comments if you are interested by a set of posts to answer those questions. Questions from SASSUR and DECSoS workshops and SafeComp: - Does IEC 63187 allow taking into account autonomy of subsystems? And more generally AI systems? - Does IEC 63187 allow taking continuous deployment into account? - Can dynamic assurance and continuous safety cases be used within IEC 63187? - What are the challenges that are yet to be addressed by IEC 63187? - How will realisation standards interface with IEC 63187? - What are the challenges for those realisation standards integration? - Is IEC 63187 an integration standard? - What are the links between IEC 63187 and product security? - Is defence domain really different from the other domains? What would be the differences from safety point of view? - How will be integrated elements realised using ISO 26262?
