After considerable negotiations, the NHS can reveal the location of a new potential site for a brand new hospital to replace Royal Lancaster Infirmary.   The proposed site is Bailrigg East, situated north of and in close proximity to Lancaster University.   Proposals for investment in Furness General Hospital’s infrastructure are also in development and will be shared in due course.   This comes in addition to the announcement of a proposed site for a brand new hospital to replace Royal Preston Hospital.   The local NHS wants to hear from patients, residents, colleagues and stakeholders about what a new hospital will mean for them and how the move to a new hospital site might impact them. Integrated Care Systems (ICSs) across England continue to face pressure around hospital bed places and moving people through hospital as quickly and safely as possible to meet demand. More importantly though, we know people recover better at home, or in the place they call home. Hospital is not the place for people who no longer need hospital care, but too many people are remaining in hospital longer than they need, making them more vulnerable to complications or deconditioning and meaning hospital beds are not available for those people who do need hospital care. We used strategic communications and behavioural change to help Bristol, North Somerset and South Gloucestershire ICS support and improve hospital discharge through the national Discharge to Assess model. I am excited to be among the many Clinicians to work out of Polegate Make Ready Centre (PMRC). It is an exciting time to be able to utilise our new- Unscheduled Care Navigation Hub (UCNH) which will provide our patients with strategically lead partnerships to improve patient-centred care. This effective implementation, among the one of five will support the increasing complexities of health care challenges. On scene, and early contact from our UCNHs to promote and support Clinicians will aid the utilisation of available pathways, promote use of developing pathways, promote joint decision-making and develop Multi-Disciplinary Team (MDT) working. The UCNHs will support and redesign patient care so patients get the right care, at the right place, and at the right time. The UCNH will provide: - Knowledge for interdisciplinary treatments - The combination of combining different disciplines and fields of study to better patient care  - Alternatives to operate and complement existing pathways and healthcare systems #workingtogetherdifferently #involvingpeopleintheirownhealthandcare #enablingchange  #evolvinghealthcaresystems South East Coast Ambulance Service NHS Foundation Trust

Our increasing resources as a service are enabling positive changes and providing cross functionality for creating and maintaining a focus to bring our patients to the centre of care systems. These UCNHs are providing initiatives for in service implementations which will aid assessment, triaging and the overall process of key referral pathways. This will not only help integrated care systems already in place; however, it will strengthen, develop and promote National Health Service (NHS) pathways, and improve the efficiency of the overall NHS. As a Paramedic I'm analysing on scene whether: - My patient needs conveyance to hospital, whether this is urgent, could conveyance be delayed, whether the patient can be referred back into Primary and/or Secondary care, and whether they need any referrals completing  - What pathways are available  - What else can I do as a Clinician to utilise existing clinical care pathways - What else can I do for my patient  - Holistic care can be introduced As a Paramedic I need to: - Promote shared decision making to protect specific patient groups to provide tailored management of care to avoid deterioration - Think about specific patient groups, for example: individuals with long-term health conditions, older adults, and rare disease patients. Use of the UCNHs will contribute to the prevention of and deterioration of these patient groups especially coming into winter; as well as support pressures of the NHS. The initiative is to: - Improve patient outcome - To utilise better communication between Clinicians on scene and in the hub to support complexities of conditions and situations to better patient care and outcome - Improve knowledge sharing between different scopes of practice  - Improved treatment decisions - Improve job satisfaction  - Develop, redesign and manage resources for overall service efficiency As an evolving service we are continuing to establish and provide new rationales for effective patient care. Moving forward, as Clinicians we can break down the barriers to: - Communication and coordination on scene between accessing MDTs and pathways - Increasing resources  - Joint needs assessment  - Care planning  - Transitions of care - Accessing community-based services -Improved patient engagement -Improved patient capacity  -Improved clinical assumptions  -Improved utilisation of patient and clinician autonomy in joint partnership with developing pathways #workingtogetherdifferently #involvingpeopleintheirownhealthandcare #enablingchange  #evolvinghealthcaresystems South East Coast Ambulance Service NHS Foundation Trust

NHS Confederation chief executive Matthew Taylor responds to the new research by the Liberal Democrats on NHS hospital buildings: “With no further capital funding announced in the spring budget above what was already announced in the autumn statement, we fear that the deterioration of the NHS estate will continue. For the NHS to make the productivity and efficiency improvements needed to tackle the waiting list backlogs and recover performance, as well as ensuring that buildings are kept safe for staff and patients, it needs the right investment. This includes longer funding settlements so leaders can plan for the future, and a step away from short term thinking. Under the leadership of CEO Sandra Daly, the dedication of our team at Mater Private Hospital in Cork has driven significant progress in our healthcare offering for the Munster region. Over the past decade, through €30 million in investments, our Cork hospital has expanded services, doubled its bed capacity, and now treats over 74,000 patients annually.
