I recently encountered some concerning cases related to the use of axillary bypasses in vascular surgery. Despite the availability of advanced endovascular tools and techniques, I have come across instances where patients were offered axillary bypass as their only option, even as frontline treatment for younger patients. At one California healthcare system, it's considered frontline treatment because "It lasts longer than a TPA" followed by thrombectomy, according to their top surgeon. But what happens when that fails? What are the options for a 50+ woman who only forgot to take her anticoagulants for 2 days and has fresh occlusive clot? Ultimately, we helped her get a second opinion and the TPA/thrombectomy worked, but because the surgeon left her sitting in the hospital for five days to come into agreement with him about the only option he provided her (axillary), she ended up with a terrible infected bed sore on her buttocks and lasting nerve pain in her leg and foot from lack of blood flow for so long. In another instance, a patient was led to believe that axillary bypass was a quick and easy alternative to thoracic bypass without being fully informed about the associated mortality rate and limited options in case of failure. This lack of transparency led to misconceptions about the procedure and its potential short and long-term implications for the patient. She walked out of the procedure thinking the axillary is a permanent long-term solution until she read the study I attached below and came to us asking about it. The third patient died within the month following the axillary, which was performed despite a 70% axillary stenosis distal of the graft. Her cardiologist said the graft was already stressing the heart within days following the procedure and asked why they didn't try endovascular first. He told the daughter, endo would've been less strenuous for a patient with a lower EJF. But the surgeon said they didn't know how and neither did anyone else on the team. Key questions: What are the appropriate indications for performing axillary bypass, and how should its patency rate and potential failure be discussed with the patient? In cases where axillary bypass is considered the only option, how can we ensure that patients are fully aware of its limitations and the importance of close monitoring? What role should less invasive endovascular techniques play in the decision-making process? Yes, the axillary may have greater durability than a TPA and thrombectomy that follows, but does the risk outweigh the benefits when endovascular is an option? This study reviewed a case series of patients undergoing Immediate Sequential Bilateral Cataract Surgery (ISBCS) with the implantation of an affected batch of EyeCee One intraocular lens (EC1IOL) causing high intraocular pressure (IOP) as reported by the The Medicine and Healthcare products Regulatory Agency (MHRA). They found a greater rate EC1IOL related IOP spikes (IOP ≥30 mmHg at a post op visit) than the MRHA reports. There was a trend towards less visual gain post operatively in patients who had a spike. The benefits of Intracapsular tonsillectomy. A total of 12,275 patients were included in this study. The overall post-tonsillectomy bleed rate was 3.3 %, with 0.65 % requiring return to theatre. ICT had a significantly lower bleed rate of 1.7 % compared to 4.1 % for ECT (p < 0.001). The rate of return to theatre was markedly lower for ICT (0.08 %) compared to ECT (0.93 %, p < 0.001). Median day of bleeding was not different between the techniques. Patients undergoing surgery for recurrent tonsillitis had the highest rate of postoperative haemorrhage (15 %), while those with sleep-disordered breathing alone had the lowest (3 %, p < 0.001).
