Hi everyone Hope all doing good a I am very well sure that everyone is aware of the term Barrett's Esophagus converting into adenocarcinoma as in our case it was referred by gastroenterologist presented with complaints of swelling in the stomach, pain and watering in the mouth. Endoscopy is performed and it revealed the cervical and thoracic esophageal mucosa appeared normal, and the Z line was 40 cm distally. The esophageal mucosa had a hyperemic appearance. Multiple biopsies were taken from these areas. In histopathological evaluation special columnar epithelium mucinfilled cytoplasm, goblet cells were observed. Goblet cells were positively stained by PAS, Alcian Blue and Mucincarmine histochemical staining. Karyotyping was done which shows 50,XY,del(1)(q32q42),q23),(q31),add(3)(p21),(q23),+8,+12,(14)(q13),+20.. A very interesting case was operated recently. This lady in her late 60s was routinely scanned after undergoing right hemihepatectomy for HCC around 10 years ago. A nodule measuring 0.9x0.6cm in the right upper lobe was seen in Jan. A PET scan was done in Feb and the lesion increased in size to 1.5x1.2cm with localised uptake. A biopsy showed SMALL CELL CA. MRI brain was normal. She underwent right uniportal VATS upper lobectomy and lymphadenectomy in March and discharged well. The HPE showed the size of the lesion was 3.5x2.5cm and it was a combined SMALL CELL CA with ADENOCA. All the lymph node stations 2,4,7,8,10 and 11 were not involved. From Jan to March (~3 months), the lesion increased from 0.9cm to 3.5cm, almost 400% increase in size. DO NOT DELAY!! I have reviewed around 5 or 6 small cell ca patients but this is my first small-cell lung ca operated after around 8 years practising as Thoracic Surgeon. The “swirl sign” seen on a CT scan of the abdomen and pelvis is a radiologic finding that is significant in the context of intestinal volvulus. Volvulus refers to the twisting of a segment of the intestine around its mesenteric base, which can lead to bowel obstruction, ischemia, and potential bowel necrosis if not promptly managed. The swirl sign specifically refers to the appearance of mesenteric vessels that are twisted and rotated, creating a swirl-like or “whirlpool” pattern on imaging. This sign is most commonly associated with midgut volvulus, though it can also be seen in sigmoid volvulus.
