Fantastic News! 🌟 We’re thrilled to announce that Kooth Digital Health has been shortlisted for our project providing Universal Preventative Mental Health Support for ages 10 and up in Greater Manchester, in the most impactful partnership in preventative healthcare category at the prestigious HSJ Partnership Awards 2025! We’ve partnered with Greater Manchester Integrated Care Partnership and Transport for Greater Manchester to provide universal access for early mental health intervention for 2.5M+ people. Through a three-month campaign, we promoted Kooth (CYP) and Qwell (adults), driving healthy habits and digital support awareness. This resulted in 34,000+ logins from 10,000+ CYP and 5,000+ adults and professionals. These awards are the gold standard for recognising innovative and impactful partnerships with the NHS, celebrating initiatives that drive real change in healthcare delivery. Out of over 280 entries, we’re honoured to be selected among the exceptional finalists. We’re continually driven to deliver accessible, effective digital mental health support for all young people, so no one goes without access to mental health support. So we’re proud to have our work in Greater Manchester, recognised by the HSJ. This Co-production Week, I’m excited to share the progress Access Elemental Social Prescribing have made alongside NHS England. Having worked closely on this project and realising its potential, we are coordinating efforts to help PCNs and ICBs leverage new funding opportunities to transform primary care. Our latest article highlights how this interim funding, introduced after the Digital Pathway Framework Funding cancellation, enables PCNs and ICBs to modernise general practice by focusing on community-specific needs. With this week being co-production week, we wanted to highlight some of the benefits co-production approaches provide to improve outcomes, increase community health and wellbeing, and reduce pressure on primary care settings and GPs. Our latest article highlights a great opportunity for PCNs and ICBs to collaborate on Modernising General Practice with the new interim funding following the cancelation of the Digital Pathway Framework Funding. Here this collaboration enables PCNs and ICBs to utilise the funding effectively by catering their expenditure to what their community needs, to concentrate on what matters to the individual. One of the most impactful ways to use this funding is through improving your care navigation by investing in digital social prescribing to reduce GP workloads, address the social determinants of health, improve patient outcomes, and most importantly promote and support self-care. We are absolutely thrilled to have been named alongside Mersey Care NHS Foundation Trust and The Life Rooms, as a finalist in Health Tech News's awards. Mersey Care NHS Foundation Trust has integrated with Access HSC’s social prescribing tool with EPR to transform mental health services. The trust covers a wide geographical area including some of the UK’s most deprived communities, with community-based services deemed to be of great importance; however, a disconnect existed between health and community care with mental health clinicians unable to refer into Life Rooms, a service set up as a ‘social front door’ to provide support with issues such as loneliness and housing. Mersey Care worked with Access HSC to integrate its Rio EPR system with a social prescribing tool, Elemental, allowing mental health professionals to refer patients to Mersey Care’s Life Rooms service directly through familiar Rio software. The interoperability of systems enables service users to be supported in a safe, well-governed way by a team at Life Rooms with specialist knowledge, whilst clinical teams would also have full oversight of case notes and the support that referred patients had utilised. To date over 1,396 individuals have been referred and over 2,000 social prescriptions co-produced. Surveys undertaken indicate that prior to the introduced interoperability of systems, the average amount of time spent meeting social needs of service users was 27 minutes per appointment; since having access to this service, the average time is eight minutes per amount. S97 percent of staff reported that they felt confident meeting social needs of service users and 91 percent felt it beneficial to access social prescribing notes through the EPR. Looking ahead. 28 clinical teams across the region are now making referrals through the Rio/Elemental integration, with staff in Liverpool and Sefton recently been given access to the system. Life Rooms staff intend to use positive findings from the project to take the idea to more clinical teams nationwide. Last week I had the amazing opportunity to attend #NatCon24, the annual conference organized by the National Council for Mental Wellbeing. It was energizing to hear how CCBHCs, CMHCs, state agencies, and other organizations are working together to provide mental health and substance use care in their communities. Some key takeaways: --- Community-based care models can meaningfully impact healthcare costs. States like Missouri, New York, and Texas saw that their CCBHCs can greatly reduce hospitalizations and ED utilization, which are high drivers of cost of care --- It takes a village. Care does not happen in one setting. It takes coordination of services and resources to engage patients in their treatment and recovery journey. --- Tighter integration of primary care and behavioral health services can help patients get care faster. This is already happening at many FQHCs across the country, but more can be done in other care settings --- Payment models matter. Prospective payments allow organizations to make meaningful investments in the people and technology resources needed to provide effective patient care. PPS financing for CCBHCs enables more innovation, workforce development, and comprehensive wraparound services for whole-person care There is a lot of work to do still, I'm excited to partner with other leaders in this space to continue to advance mental health and substance use care.

The team will be using Access Elemental Social Prescribing, in addition to their previous Access Rio solution. About Berkshire Healthcare NHS Foundation Trust: They are an NHS Community and Mental Health trust, providing a wide range of services to people of all ages living in Berkshire. In a programme described as “world leading,” all 31 Health and Social Care Partnerships have now signed up to the Distress Brief Intervention (DBI) service which supports people experiencing distress. Frontline staff in Police Scotland, Scottish Ambulance Service, Accident and Emergency Depts, Primary Care and NHS 24 can refer people who present to them in emotional distress – and if assessed as appropriate - to third sector organisations which provide compassionate and personalised support. People in distress who speak to a call handler at NHS 24, Police Scotland or the Scottish Ambulance Service can also be referred for DBI support, from anywhere in Scotland. NHS 24’s Medical Director Dr Ron Cook said, "The DBI program has empowered the 111 Mental Health Hub at NHS 24 to offer a transformative pathway for callers, fostering collaborative and compassionate care journeys. Witnessing the positive impact on thousands of lives and collaborating with local teams to provide timely, personalised support across Scotland is truly rewarding. This partnership not only empowers our staff to confidently offer this vital service, but also equips individuals with the tools to both reduce immediate, and manage future, distress.
