Transcatheter aortic valve replacement (TAVR) can be performed safely in adults with congenital heart disease (CHD) if done by a multidisciplinary team of specialists at a high-volume center, so concludes a Cleveland Clinic case series. TAVR can be an effective intervention in CHD in select clinical scenarios, proving both feasible & safe with good short-term outcomes. We are very pleased to learn of the publication of this recent journal article in the British Journal of Surgery, which evaluated the use of LumenEye in place of flexible sigmoidoscopy for patients referred with anorectal symptoms to the colorectal outpatient department. The authors conclude that LumenEye X1 'offers a potential solution to helping to relieve the endoscopy burden and enable cost savings by reducing the number of outpatient flexible sigmoidoscopies'. 'In patients with a clear DVT-provoking risk factor, such as after surgery, the pick-up rate for further investigations for cancer will be low and probably not cost-effective... 'However, all patients presenting with a unprovoked DVT should have a screening history and basic physical examination to check for underlying cancer or red flag symptoms... 'Patients with an unprovoked VTE should be screened for anti-phospholipid syndrome as this will affect anticoagulation choice, the need for other anti-thrombotics, the duration and dose of anticoagulant therapy.' 📃Scientific paper: Is endourological intervention a suitable treatment option in the management of iatrogenic thermal ureteral injury? A contemporary case series Abstract: Background Iatrogenic ureteral injury (IUI) is relatively rare, however, can cause sepsis, kidney failure, and death. Most cases of IUI are not recognized until the patient presents with symptoms following pelvic surgery or radiotherapy. Recently, minimally invasive approaches have been used more frequently in the treatment of IUI. This study evaluates urological intervention success rates and long-term clinical outcomes according to the type of IUI following hysterectomy. Methods Twenty-seven patients who underwent surgery due to IUI in our clinic following hysterectomy were evaluated between January 2011 and April 2018. Patients were classified according to the time of diagnosis of IUI. The IUI cases diagnosed within the first 24 h following hysterectomy were designated as "immediate" IUI, while that diagnosed late period was considered 'delayed' IUI. The type of IUI was categorized as "cold transection" if it was due to surgical dissection or ligation without any thermal energy, and "thermal injury" if it was related to any energy-based surgical device. Patient information, laboratory and perioperative data, imaging studies, and complications were assessed retrospectively. Results All cases of delayed diagnosis IUI were secondary to laparoscopic hysterectomy ( P = 0.041). Patients with thermal injury to the ureter were mostly diagnosed late (delayed) ( P = 0.029). Swift and Dedicated Care Saves Brain Hemorrhage Patient with Blood Cancer in Critical Condition - The hospital’s first case to combine burr hole surgery and middle meningeal artery embolization, a groundbreaking treatment for recurrent chronic subdural hematoma. The patient was diagnosed with essential thrombocytosis, a condition causing excessive platelet production and abnormal blood clotting. He had recently developed a chronic subdural hematoma, a condition where blood gradually accumulates between the brain and the dura mater. He initially underwent burr hole surgery under local anesthesia, involving a small 3 cm incision to drain the blood, and showed improvement. Title: Utilization of Indocyanine Green (ICG) Fluorescence in Patients with Pediatric Colorectal Diseases: The Current Applications and Reported Outcomes 🔍 Highlights: • This review synthesizes findings from January 2010 to March 2024, evaluating ICG's role in enhancing surgical outcomes for Hirschsprung disease, anorectal malformations, cloacal reconstructions, vaginal agenesis, bladder augmentation, and antegrade continence channels. • ICG fluorescence significantly aids intraoperative decision-making by improving visualization of vascular networks and assessing tissue perfusion. • Despite the limited number of studies, initial findings indicate that ICG may offer advantages over traditional clinical assessments for intestinal perfusion with a promising safety profile in pediatric patients. • The use of ICG could enhance surgical precision and patient recovery by providing real-time, accurate perfusion data. • Larger, prospective studies are needed to validate these observations, quantify benefits, and further assess its impact on clinical outcomes. 📈 Conclusion: ICG fluorescence imaging can revolutionize pediatric colorectal surgery by ensuring optimal perfusion during complex procedures, thus improving surgical precision and patient recovery.
