Exposing the many intricate levels of social determinants of health highlights systemic inequities deeply rooted within society, necessitating a comprehensive approach beyond conventional healthcare interventions. We have to address the pivotal role of market access in addressing these disparities. Addressing and working to dismantle them promotes community health and strengthens the fundamental pillars of societal welfare, shaping a future where equitable healthcare access and opportunities are the norm for all individuals. We are working on these inequities at Perigee Market Access through our proprietary Disease Dashboards™ solution. Disease Dashboards uses advanced analytics, machine learning and artificial intelligence (AI) technologies to identify and evaluate health inequities in the disease-specific patient journey. #MarketAccess #Healthcare #SDOH #DiseaseDashboards

While algorithms in health care aim to reduce inequality and improve health care outcomes, a University of Pennsylvania study found race is often an unreliable variable in determining care, as it is always a proxy for something else, such as genetics or social determinants of health. In fact, some algorithms may still be contributing to disparities in care. These algorithms are only as good as the data that underlie them, necessitating mindful and transparent development of these tools. https://lnkd.in/e4n-Qh_u #digitalhealth #hygieia #healthdataequity #Philadelphiahealth

In conversations with #CommunityHealth leaders and #HealthEquity experts, one thing is clear: there’s immense potential to enhance #efficiency and #access in community-initiated care with #AI. Over 100 million people including seniors, families, and children have complex care needs funded by Medicare, Medicaid, and CHIP. But here is the challenge: 🫂 600,000 "care coordinators" across health and public health systems are tasked to provide the knowledge, resources, and referrals needed for holistic care—spanning medical, dental, food, housing, and more. 🏋♂️ For this system to truly work, each care coordinator would need to manage 300 people. Without AI, even today's caseloads are proving to be #unsustainable, resulting in burnout, and people falling through the the Swiss cheese in our safety net. #AI can't do everything, but it can augment these dedicated human workforces, while providing greater engagement and support for individuals. Imagine if an organization with a deep local network could harness its intelligence and magnify their impact. Could we lift the quality of life? Could we reduce burnout and empower coordinators to do what they do best—to connect and guide care? That is the question. #AIForGood #HealthcareInnovation #SocialImpact #CareCoordination #CommunityHealth #MedicaidInnovation #ValueBasedCare #HealthTech #PopulationHealth #HealthEquity

Achieving health equity is a challenge, especially when your zip code can determine how healthy you are. As can your race, gender, sexual orientation, or education level. Social determinants of health such as these, drive 70% of health outcomes—but there are ways to close the gap. Quality improvement provides an opportunity for health plans to identify and address healthcare disparities tied to social determinants of health. With Arine's AI-powered medication intelligence platform, clinical teams get insights into the member’s specific needs, allowing them to more effectively tailor their interventions and address any barriers the patient may have to getting the right care. It is through increased personalization like this that we will get closer to addressing healthcare disparities and achieving health equity. 👀Read our latest blog post to learn more. https://hubs.li/Q02DMYNJ0 #QualityImprovement #HealthEquity #SDoH #AI #MedicationIntelligence
