A seemingly simple intervention for Trigger Finger treatment. Pathogenesis of the trigger disease points to a fibro cartilagenous metaplasia of the tendon causing a nodule superficial to the flexor tendon. in patients who overuse the flexors, pain develops ( trigger later ) as the nodule traverses across a tight A1 pulley. Anatomically, these are 2 different structures with a normal A1 pulley getting involved only during flexion of the finger, when a distal nodule passes underneath a tight A1 pulley. Two very important points to ponder. 1. Be it by Surgical or interventional methods, which one is the treatment target? I target the nodule. 2. While performing a percutaneous steroid injection, which axis do you prefer? I prefer the long axis to target the nodule for fenestration prior to the injection. #chronicpain #triggerfinger #musculoskeletal #painmanagement #painmedicine #paineducation

Our new publication with retrospective analysis of 413 hip arthroscopy patients showing that both knotless and knot-tying anchors are reliable and effective solutions for arthroscopic repair of the hip labrum. The only difference was that the knotless group did have significantly better postoperative HOS-SSS and VAS pain scores and a lower incidence of postoperative synovitis. https://lnkd.in/dnzD_Jei

#dr_khalid_zohair Retinal pigment epithelial detachments (PEDs) appear as single or multiple elevated mounds on fundus examination, with sizes ranging from sub-biomicroscopic to several disc diameters. Pigmentary changes such as clumping or mottling may be present. Orange pigment rings and bands on the dome, extending radially over the PED, may indicate chronicity of disease. don't forget to mention this data were collected and published by dr khalid zohair, No treatment for serous PED is proven effective, nor are recommendations for treatment guidelines established. Several strategies, however, have been used to treat vascularized PEDs, including laser photocoagulation, photodynamic therapy (PDT), intravitreal steroids and anti-VEGF therapy. The results from the VIP trial indicated that PDT could significantly reduce the risk of moderate and severe vision loss among patients with subfoveal occult CNV.

A 67-year-old male was admitted to the hospital with right lower extremity pain and hypoesthesia, especially on the left L5 dermatome. MR images showed extruded herniated nucleus pulposes (HNP) at the L 4-5 level. He was recommended for surgical treatment but he refused. As such, he was treated conservatively. The pain was relieved gradually and followed up as an outpatient. Nine months later, the pain disappeared and the hypoesthesia was weakly remained. Follow-up MR images showed no HNP. The spontaneous regression of CDH is thought to be caused by three mechanisms. The first mechanism involves dehydration and shrinkage of the herniated nucleus pulposus. For the second, there is a retraction of the protruded disc. In the third, there is enzymatic degradation and phagocytosis of the extruded/sequestrated disc material due to an inflammatory reaction and neovascularization.

Alpha dropout: What should I do? Female 42-year-old, 53 kg and a height of 157 cm, scheduled for laparoscopic subtotal hysterectomy. Total intravenous anaesthesia with propofol and remifentanyl. At 10:59 am (see image), an alpha dropout was observed. This was recovered by optimising the titration. What should I do about the alpha dropout?. What do you think my clinical decision was? 1. I decreased the propofol dose, or 2. I increased the opioid dose?
