As caregivers, witnessing such transformative outcomes fills us with immense happiness and fulfillment. it is a journey of restoring vision and changing lives! and here another Round with #ChatGPT: White mature cataracts present unique challenges during surgery due to their dense and opaque nature, which can make it difficult to visualize the structures within the eye. Some of the challenges include: 1. Limited Visualization: The dense opacity of the cataract can obstruct the surgeon's view, making it harder to accurately assess and manipulate the affected lens. 2. Increased Risk of Complications: White mature cataracts are often associated with weaker and more fragile lens capsules, increasing the risk of intraoperative complications such as capsule rupture or posterior capsular opacification. 3. Capsular Integrity: Maintaining the integrity of the lens capsule is crucial during surgery to support the intraocular lens (IOL) implantation and ensure optimal visual outcomes. 4. Inflammation and Swelling: Due to the increased manipulation required to remove the dense cataract, there is a higher likelihood of postoperative inflammation and corneal swelling, which can affect visual recovery. Why Are We Still Choosing Open Surgery Over Laparoscopy? Lately, I’ve been seeing more and more patients who have undergone open surgery for conditions that could have been managed laparoscopically. It’s concerning—and it raises an important question: why are we still choosing open surgery when minimally invasive options are available and often better? Why is this happening? • Training Gaps: Laparoscopic surgery requires advanced skills and experience. Are we, as a community, doing enough to ensure every gynecologist is equipped to offer it? • Patient Awareness: Some patients believe a bigger incision means a better outcome. Are we taking the time to educate them about the benefits of laparoscopy? • Systemic Challenges: Limited infrastructure or support in some institutions may leave open surgery as the only option. How can we address these barriers collectively? As surgeons, our responsibility is to offer the best care based on evidence, not convenience or habit. When I see these patients, I can’t help but wonder: could their outcomes—and their experiences—have been better with laparoscopy? This isn’t about pointing fingers. It’s about reflecting on how we can do better for our patients. If a minimally invasive option is viable, shouldn’t we advocate for it—or REFER to someone who can? Are we staying true to the principle of primum non nocere (first, do no harm) by utilizing the most advanced, evidence-based techniques at our disposal? I’d love to hear your thoughts. Happy to share that the first paper eliciting patients’ preferences regarding Bone-Anchored Protheses treatment, outcomes, and related complications 🌟 & First collaboration between the Osseointegration Center Rotterdam (OCR) and Osseointegration Nijmegen group titled: Patients' Preferences for Bone-Anchored Prostheses After Lower-Extremity Amputation: A 2-Center Discrete Choice Experiment in The Netherlands (PREFER-BAP-1) has recently been published in The Journal of Bone and Joint Surgery! https://lnkd.in/eEpZRguN 🔑 Key Insights: 🔹 Long-Term Complications Matter Most—3.4x more than short-term risks. 🏥 🔹 Quality of Life is a top priority, second only to avoiding long-term issues. 🔹 Costs Are Critical—out-of-pocket expenses significantly impact decision-making, opting out due to no reimbursement is a burden. 💸 🔹 Implant Removal has the highest QOL loss (1.15) and financial loss impact (€16,940). 🩺 Implications for Care: Patient preferences should be at the forefront of treatment decisions.
