First case only over month ago. Here is our second case in Nigeria!!! Deep vein thrombosis is the formation of clot in one of the veins in the leg. This can be caused by multiple factors including a high propensity to clot from several factors or long term immobility. When a clot forms in the leg, this typically manifests with leg swelling and should promt you to seek medical attention. Most patients are treated successfully with blood thinning(anticoagulation) medications. It is important that you stick to the anticoagulant regimen to avoid formation of large clots in the big veins in the belly(inferior vana cava). If this happens, it becomes a serious medical problem. We can however still treat this patient’s successfully here in Nigeria with intervenal radiology techniques. #iliocaval thrombosis Euracare

The patient gave a history of chronic liver disease and band ligation for esophageal varices. Laboratory studies revealed thrombocytopenia, PLTcount= 38000. the patient underwent an upper digestive endoscopy. The scope revealed esophageal varices and band ligation was performed. One minute after withdrawal of the scope, there is an attack of fresh bleeding. Upper endoscopy was done and revealed oozing around the rings of band ligation https://lnkd.in/dZEkiqe6

Gastroparesis crash course # 2 How do you work it up? Do upper endoscopy Why? The symptoms of gastroparesis mimic those of pyloric obstruction. So, anything causing obstruction at the pylorus, like a tumor or pyloric stenosis, would mimic gastroparesis. EGD done --> pyloric obstruction ruled out --> now what? Do one of the following 3 studies. 1. Nucear medicine gastric emptying study, OR 2. Wireless motility capsule study, OR 3. Gastric emptying carbon breath testing study Keep in mind that if the patient is diabetic, the blood glucose has to be less than 275 mg/dL because hyperglycemia can itself impair gastric emptying. 🧡 How #Memosorb series of biodegradable occluders are shaping the future of cardiac care ❤️ #Research Sharing: First-in-Human Experience With a Novel Fully Bioabsorbable Occluder for Ventricular Septal Defect The first human experience with the implantation of fully bioabsorbable occluders for VSDs was performed using echocardiographic guidance and a specialized shaping line. Between February and June 2018, five patients with perimembranous VSDs underwent successful closure at Fuwai Yunnan Cardiovascular Hospital. The fully bioabsorbable occluders, with a polydioxanone framework and poly-L-lactic acid fabric were implanted using a hybrid perventricular approach. After occluder deployment, a “double-umbrella” framework shape is achieved by pulling back the line while pushing the delivery cable. With a mean defect size of 5.1±0.5 mm (range: 4.5 to 6 mm), mean occluder size of 7.4±1.1 mm (range: 6 to 9 mm), mean total procedural time of 59.0±11.2 min, and mean post-operative hospital stay of 2.8±0.8 days (range: 2 to 4 days), the fully bioabsorbable occluders were implanted successfully in all patients, without occluder malposition, atrioventricular block, residual shunt, or valvular regurgitation. All patients completed 6-month follow-up, without deaths or severe complications such as aortic regurgitation, embolism, and malignant arrhythmias. Today, the MemoSorb series of biodegradable occluders has been used in over 3,000 cases across China, offering a healthier and safer future for patients with structural heart disease. This article is for you!🚨 A fascinating study recently published in *RAPM* sheds light on the complexity of adipose tissue compartments enclosed by fascia and how they impact nerve block outcomes. 🔍 Using postmortem histology and in vivo micro-ultrasound, the study reveals that local anesthetic agents can occupy multiple, non-communicating fat compartments, leading to variability in: - Block success rates - Onset time - Block density - Duration of effect - Secondary continuous block failure Why does this matter? Classic nerve block mechanisms and histology do not account for these adipose compartments. This study suggests that these fatty compartments may also influence injection pressures, challenging our assumptions about needle tip location during procedures. I recently had the opportunity to perform a challenging yet rewarding procedure on a patient who developed a renal arteriovenous fistula following a motor vehicle accident. This rare vascular malformation, characterized by an abnormal connection between the renal artery and vein, can lead to significant complications if left untreated. Using the Onyx™ Liquid Embolic System by Medtronic, we successfully embolized the fistula, preserving renal function and preventing further complications. The advantages of liquid embolic agents, such as controlled delivery, excellent visibility, ensuring complete filling, and distal penetration of the vessels, make it my first choice in trauma with bleeding. This case underscores the importance of timely intervention and the advancements in interventional radiology that allow us to treat complex conditions with minimal invasiveness.
