🔎 'The Dutch Safety Board is working as fast as possible, but also as slowly as necessary.' In the quarterly aviation report for the second quarter of 2024, the Safety Board highlights the on-site investigation. Aviation investigators have been on site several times in the past quarter in response to reports of incidents. They examine for each occurance an on-site investigation is of added value; In doing so, they look at the seriousness or extent of the occurance. On-site investigations require a balancing of interests: it is done as quickly as possible, but also as slowly as necessary. Because as important as the investigation of an aviation accident/incident itself is how it is written, how it is transmitted to the public. The Final Report must be clear, concise and logical; It must be carried out in a completely transparent manner. The public deserves to know when, how and why the accident occurred. This is only possible if the official investigative bodies of the States comply with the basic procedures described in ICAO document 9756 Aviation Accident and Incident Investigation Manual, part IV of Report Writing. In high-risk industries such as aviation, incidents and accidents provide crucial data for uncovering latent hazards, system vulnerabilities, and organizational deficiencies. Failing to conduct thorough investigations not only hinders the organization's ability to address these vulnerabilities, but also perpetuates systemic risks that can lead to recurring accidents. Effective investigation involves the comprehensive collection, analysis, and interpretation of data to identify root causes, human factors, and technical failures. This information informs risk-based decision-making, enhancing safety management strategies, improving safety culture, and optimizing resource allocation for safety-critical interventions. Inadequate investigation represents a missed opportunity to bolster operational resilience and the organization's capacity for adaptive learning. Rigorous investigation methodologies reinforce the concept that safety is a dynamic process requiring constant vigilance, learning, and evolution. Essentially, missed opportunities for learning compromise the organization's ability to prevent future incidents, impacting safety outcomes and regulatory credibility.

Robust accident investigation protocols are essential to global aviation safety. “Less than half of the required #aviation accident reports meet the critical standards of thoroughness and timeliness—a direct violation of the Chicago Convention. It’s imperative that governments uphold the rigorous accident investigation protocols of ICAO Annex 13. Completing and publishing these reports isn’t optional; it’s a crucial obligation to enhance global aviation safety”, says Gabriel Acosta, IATA's Head Operational Safety.
