Is a dentist considered a high-cost worker for the health system? Some area in Ministry of Public Health in Thailand has pointed out the challenge of the high cost associated with employing dentists, which may not yield significant profits. One senior doctor suggested hiring dental hygienists as a more cost-effective alternative. This issue is compounded by the inadequate infrastructure for dental care in the public sector, where the number of dentists often exceeds the available dental units. In addition, the calcution of workload and dental costs have not been revised and adjusted to the current situation. A study from 2011 revealed that around 52% of Thai dentists retained in the public sector, but this number shall be declining. Solving these issues requires a systemic approach, possibly involving restructuring budgets and investing in dental infrastructure. However, it may also necessitate addressing hierarchical structures among professionals within the Ministry of Health to enact meaningful change, since the workload and areas of work between medical doctors and dentists are often issues for comparison at various stages.

CORPORITIZATION OF MEDICINE AND DENTISTRY is by far the greatest threat and challenge we face as Clinicians in this century throughout our history of thousands of years as Doctors (and it is a dangerous form of betrayal to enable or empower them). Say GOODBYE to the endangered species known as the Dental Service Organization (DSO). Say HELLO to artificial intelligence (AI) which already does everything they do minus the damage to our reputation and the modern day slavery. Once again, Dentists will have true ownership of their practices, their own equity, and their own autonomy. The two main lies of the DSO are a moot point now and should be consistently responded to as follows: A. “We increase access to care.” NO SIR, Dentists increase access to care. B. “We only provide management services for Dentists.” NO SIR, you bribe others, circumvent the law, damage our reputation for profit and control the operation(s) directly or indirectly, so you and your fat cat friends can profiteer off Dentists and Patients’ backs. HAVE SOME SELF-RESPECT. DON’T BE THEIR SLAVE. CUT YOUR TIES WITH THE DSO AND LIVE UP TO YOUR HIPPOCRATIC OATH. #DoctorToDoctor

KNOWING YOUR LIMITS AND CASE SELECTION!! You need to perfect this one skill if you are a new dentist. Knowing your limits and case selection can make your life or literally ruin it. It is your responsibility to perfect this skill not only for yourself, but for your patients. I always believe in: "I know what I know and I know what I don't know. And what I don't know, I know who does". I have learned a lot in the past few years and this skill of learning what my limits are has been INCREDIBLY IMPORTANT! Now, I can see things more clearly. Just because you can do a procedure and do it well, mean that you want to perform it on every single patient? Somebody who has limited opening will impact your visibility, which could ultimately affect the final restoration. Someone with a complex medical history may not be a candidate for certain treatments. I can do root canals but for the ones that have a higher chance of complications I will refer to the endodontist. Specialists are here for a reason! These days, I am actually doing more procedures but also referring out more than I have in the past. One thing I have experienced is I can see the potential of every patient. I am now offering more things to patients, I recommend the procedures that I think may benefit them. When the patient goes to see a specialist and they have a good experience, they have more trust in you for sending them somewhere nice.
