From integrating care teams to leveraging data-driven decisions, value-based care is revolutionizing healthcare. “We entered into value-based care about 20 years ago. At that time, it stemmed from our belief that we need to provide better coordination of care rather than having a transactional relationship with the patients,” explained Dr. Adnan Munkarah, our chief clinical officer & president of care delivery system. “We have learned over time that in order to provide value-based care, you need to have the right team. You want to make sure that you are providing the access, and you need to engage patients.” A key element of our approach has been working with nonphysician providers, medical assistants, care managers and other health professionals to help establish as many access points as possible for care. Accessible healthcare for young people is essential and here at PP we work with so many wonderful partners, such as the Care Quality Commission, to make that a reality! But how exactly do we do that? 🤔 We team up young people and senior leaders and together they tackle big challenges facing the healthcare industry. This can include reaching and engaging Gen Z and Alpha, helping young people transition smoothly from paediatric care to wider health services, or simply empowering them to take charge of their health. The young people get a chance to have their voices heard and the senior leaders, equipped with their valuable insights, have actionable strategies to make a difference! This photo was taken earlier this year at one of our CQC Young Champion meet-ups where young people came together to discuss issues that affect them. They looked at waiting times, technology in healthcare and communication with medical professionals. Afterwards, they talked about solutions to these challenges and left them in the trusty hands of the CQC. When we work with young people, for young people, we know exactly how to serve them and can make the most out of our efforts.
