In the current era of artificial intelligence, it is important and often difficult to distinguish hype versus substantive impact. This is very true in our field of acute stroke as well. When every minute counts until the artery in the brain is opened up and blood flow restored, we are always searching for ways to optimize workflow. Most of the delays to stroke thrombectomy that exist today are due to pre-cath lab bottlenecks and process inefficiencies. It is critical that we continue to work with EMS to streamline pre-hospital assessment and triage of suspected large vessel occlusion stroke patients. Take, for example, an EMS stroke alert patient that arrived by Pembroke Pines Fire Rescue last week highlighting the essence of parallel process. Patient in his 50s with left gaze preference, aphasia, right hemiplegia who arrived during after work hours: 1) Due to high RACE score on paramedic assessment our hospital was notified in advance of ambulance arrival 2) Neurointerventional team was alerted by phone call from ER prior to imaging based on high RACE score and dense MCA syndrome - a clinical presentation with high pre-test probability for LVO 3) Imaging confirmed LVO - with alert from our AI app a few minutes later; NIR physician reviewed the source imaging, due to false positives from AI 4) NeuroInterventional physician communicated the findings to ER physician - clinical judgment for patient selection is key 5) Patient transferred immediately from CT scanner in ER to cath lab as the team was ready due to early activation Outcome: Full reperfusion achieved - patient talking, lifting right arm/leg on the table Workflow Metrics: Door to puncture - 57 minutes Puncture to vessel open - 25 minutes If we had waited until our AI app alerted us with LVO detection to activate the cath lab, the sequential process would have delayed all of the steps downstream which would have significantly reduced the chance of this patient having a good outcome. An AI app is only as good as the process within which it is implemented. Quite incredible what AI for healthcare applications are capable of today... and it's still only very early days in AI. I've been mostly working in the field of radiotherapy for the past 20 years and have seen the rapid evolution of technology around treatment planning, patient imaging (or as it's referred to in radiotherapy: simulation) and treatment delivery. For example, various companies tried to automate contouring of avoidance organs for the last two decades. A multitude of approaches have been used and although for some volumes results have been quite decent, using technologies like model-based or multi-atlas segmentation, overall every vendor failed thus auto-contouring wasn't really mainstream in the past. However, the real breakthrough came just a few years ago when the first AI auto-contouring solutions came on the market resulting in a step-change in the clinical utility of the results. Human Bytes, the company I work for has been blessed to partner since 2022 with what I'm convinced is the best AI in radiotherapy supplier: TheraPanacea. I don't say that necessarily because their AI auto-contouring performance is consistently ranked as the best in recent multi-vendor comparisons, but because their speed and utility of innovation is unlike anything I've seen in my career. I like this pace a lot. But that's actually not the main reason of my post. What I was very recently also amazed by is our new partner at Human Bytes: Corti.ai. Corti understands ambient (freeform) patient conversations and clinical reporting, and transcribes it on-the-fly into a documented report including ICD-10 codes or your equivalent local codes. Oh, and it corrects the report as you correct yourself speaking. Asian gastro docs generally trust and accept AI 🗒 ✨ The survey conducted by Nanyang Technological University Singapore reveals a generally positive reception of AI among gastroenterologists and gastrointestinal surgeons in Asia. The majority of the 165 professionals surveyed expressed trust and acceptance of AI in diagnosing and assessing colorectal polyps, with about 80% in favor of AI for detection and characterization, and 70% for AI-assisted removal. Interestingly, the survey also found that less experienced gastroenterologists perceived more risks associated with AI tools compared to their more seasoned counterparts. This suggests that confidence in AI may grow with clinical experience, and highlights the importance of familiarizing young medical professionals with AI technologies. The study underscores the potential of AI to enhance medical practices and patient care, particularly in specialties that rely heavily on image-based diagnosis and interventions, such as gastroenterology. As AI tools become more prevalent, their acceptance by healthcare professionals will be crucial for their successful integration into clinical settings. Are we already in the era of being replaced by AI? Many are opting out of radiology for fear of being left without a job in the near future. Meanwhile, the number of examinations is increasing, there aren't enough professionals to deliver quality results and those who are up to the challenge are overloaded with work. Radiology is a fascinating and very important medical speciality that allows for extensive clinical, teaching and research activity. May the next generations realise that AI is important, but it remains a tool that does not dispense the critical sense of experts. AI tools can simplify and improve vital workflows for providers such as fetal heart exams, pelvic floor assessments, fetal anatomy identification and 3D fibroid mapping.
