For COLPs and COFAs, maintaining a robust **breaches register** and fostering a culture of honesty within the firm can make all the difference. Using a traffic light system can simplify breach assessments: 🟢 Minimal impact, quickly rectified – document and move forward. 🟠 Trends or repeated issues – prepare to act with training or internal improvements. 🔴 Significant client impact – stop and report to the SRA immediately.
