**New Research Alert: Systematic Review and Meta Analysis on Minimally Invasive Surgery for Bunionette (Tailor's Bunion) Deformity** I'm excited to share our recent systematic review, published in Foot & Ankle Orthopaedics, and presented at the recent American Orthopaedic Foot & Ankle Society meeting in Vancouver, exploring the outcomes of percutaneous or minimally invasive techniques for the correction of bunionette deformity. Key findings: 🔹 Study Overview: Analyzed 18 studies, covering 580 patients (714 feet), focusing on clinical and radiographic outcomes. 🔹 Techniques: Most studies used unfixed distal osteotomies, showing significant improvement in radiographic angles and patient-reported outcomes. 🔹 Complication Rates: Ranged from 0% to 21.4%, with no major complications related to nonunion (0%-5.6%). The most common complication was hypertrophic callus, which resolved over time without further intervention. 🔹 Clinical Outcomes: Significant improvements in pain scores (VAS) and functional outcomes (AOFAS score). 🔹 Conclusion: Percutaneous techniques are generally safe, improving patient outcomes and radiographic alignment with fewer complications compared to open surgery. 🌟 ** My Exciting New Manuscript on Knee Surgery Outcomes!** 🌟 I am thrilled to share my latest manuscript findings on the impact of preoperative vagal activity on recovery after total knee replacement (TKR) surgery. 🔍 **Key Findings:** - Pre-surgery heart-rate variability (HRV) can predict postoperative inflammation and recovery time. - Patients with higher HRV levels experienced quicker recovery and lower inflammation levels, as indicated by C-reactive protein (CRP). - Personalized interventions, such as HRV biofeedback, could enhance surgical outcomes by improving vagal tone before surgery. 🔬 **Why It Matters:** This research highlights the potential for using HRV as a marker for personalized surgical preparation. By understanding a patient's HRV, we can develop tailored interventions to improve recovery and reduce complications. 👩⚕️ **Future Directions:** The study opens up exciting possibilities for personalized medicine, where targeted interventions could enhance recovery and patient satisfaction. ❗ To date, 10-20% of patients remain unsatisfied following total knee arthroplasty (#TKA). In response, orthopaedic surgeons are increasingly leveraging technological advancements. #Robotic-#assisted surgeries have become a significant trend in the field. Increasing evidence also supports a shift from traditional mechanical alignment (MA) to more #personalized #strategies such as restricted inverse kinematic alignment (#iKA). A recent study published by University College Cork in KSSTA Journal investigated the efficacy of a conventionally instrumented, restricted #iKA technique in restoring the constitutional coronal plane alignment of the lower limb during total knee arthoplasty. The technique was applied across a diverse patient cohort of 79 individuals, exhibiting pre-operative #valgus, neutral, and #varus alignments. A range of prostheses was utilized, demonstrating the technique's adaptability. Radiographic measurements were performed #pre- and #post-surgery using #IBLab #LAMA, ImageBiopsy Lab's tool for leg angle measurement (reproducibility of 96%). Low patient expectations regarding efficacy of PT for RTC rehab were found to be a predictor of early surgery. In patients that delay surgery AND have symptomatic full thickness tears, >70% had successful outcomes with PT and non-op management. Take home message here - let's all work to build patient confidence in non-operative management options. Positively framing an intervention can work wonders for patients. We can always perform surgery at any time, but can't reverse it once it's done! Surgery can be costly for the patient and healthcare system, and also results in missed time at work putting further strain on the patient and our economy. American medicine needs to shift focus back to conservative management and quality care. Be the change you wish to see in the world.
