Clinical trials have paved the way for life-saving therapies, transforming what was once a devastating diagnosis into a manageable condition for millions. These advancements would not be possible without the tireless efforts of researchers, healthcare professionals, and the bravery of trial participants. Organizations like the Bill & Melinda Gates Foundation and other NGOs and philanthropies have played a pivotal role in funding research and making therapies accessible to underserved communities worldwide. While we’ve come far, the journey is not over. Continued investment in clinical research is vital to reach the ultimate goal: a world free of AIDS. On this World #AIDS Day, the International Antiviral Society–USA Panel has released updated guidelines for HIV treatment and prevention, published in JAMA, and co-authored by Rajesh Gandhi, MD, and colleagues from Massachusetts General Hospital. World AIDS Day serves as an important reminder that we must remain steadfast in our commitment to prevent new HIV infections and provide essential services to all people living with HIV globally. These recommendations highlight the different treatments and prevention strategies for the disease, while also emphasizing the importance of addressing care disparities to ensure equitable access and progress toward ending the HIV epidemic. Mitochondrial Dysfunction: A Hidden Crisis in HIV Mitochondria are the powerhouses of our cells, crucial for generating energy (ATP). But when they malfunction, it leads to a cellular energy crisis with far-reaching effects. In the context of HIV, this dysfunction—referred to as HIV-Associated Mitochondrial Dysfunction (HIVAMD)—can be driven by the virus itself or by antiretroviral treatment. Key components like the Electron Transport Chain (ETC), mitochondrial DNA (mtDNA), and mitochondrial membranes can all be disrupted, leading to impaired ATP production, increased oxidative stress, and accelerated aging. The consequences? Fatigue, muscle weakness, neurological problems, metabolic disorders, and more. Understanding and addressing mitochondrial dysfunction could be key to improving the health and quality of life for people living with HIV. Just in time for World AIDS Day, I am beyond proud that HHS has finalized our HOPE Act rule, that will expand access to kidney and liver transplants for people living with HIV by removing clinical research requirements for these transplants. This action marks a significant step forward in increasing the availability of life-saving organs for all patients regardless of their HIV status while continuing to reduce barriers and stigma for people with HIV. The final rule, which further implements the HIV Organ Policy Equity (HOPE) Act, removes the clinical research and institutional review board (IRB) approval requirements for kidney and liver transplants between donors with HIV and recipients with HIV. This change is based on research demonstrating the safety and effectiveness of kidney and liver transplants between donors and recipients with HIV. Also today, in tandem with the final HOPA Act rule, National Institutes for Health published a proposed revision to their research criteria for HOPA Act transplants of additional organs such as hearts, lungs, and pancreas. Microvascular Health Solutions announces a new study using GlycoCheck that links HIV infection as a major cardiovascular risk. The study was conducted by the Athens Medical School and Attikon University Hospital in Greece. Endothelial glycocalyx (EG) derangement has been associated with cardiovascular disease (CVD). CVD is among the leading causes of mortality and morbidity among people living with HIV (PLWH), and HIV infection is now recognized as a major cardiovascular risk factor.
