Many individuals in our care have had adverse childhood experiences (ACEs). These experiences can continue to affect them throughout their lives. As practitioners in education, health and social care, what do we understand by the term ‘ACE’, and how can we best support individuals who have experienced adversity in childhood? A systematic review and meta-analysis of global ACE-related data carried out by World Psychology, and referenced in Psychiatry Advisor in December 2023, estimates that 60% of the world’s population has been exposed to at least one ACE. “Youth mental health entering dangerous phase, as experts warn of megatrends affecting young people” Today is an exciting day... The Lancet Psychiatry Commission on youth mental health was released today after four years of work from colleagues across the globe 🧠 🌍 💙 Although I am no longer working directly in the space, after a decade of studies and volunteer work with organizations like Jack.org and the International Association of Youth Mental Health, I was HUMBLED be asked to contribute to this writing as a youth co-author 4 years ago. I had a very, (I mean very) small part in this project - bringing my lens to the first section of the piece around 'The Changing Landscape of Youth Mental Health'. This brilliant, global team of experts - lead by Pat McGorry from Orygen - including youth co-authors, pulled together (through covid might I add) an incredibly impressive piece of work that paints such an important picture of the mental health of young people across the globe. While the picture doesn't look pretty, the message and call to action is LOUD. There have never been so many "megatrends" affecting young people and their mental health - investment in prevention, early intervention, and evidence-based care are urgently needed. The existing models of mental health care globally covers paediatric and adult mental health, but largely ignore the developmental needs and experiences of adolescence and young adults. In fact, there is a complete disruption within the middle of that window (age 18) that bumps young people out of paediatric care and into adult care - which is often where they fall through the cracks. The Commission calls for models of care to be redesigned around the “emerging adulthood” phase (12–25 years). Investments made need to be culturally appropriate, accessible, and align with young people's needs and experiences... and ideally, they need to be CO-DESIGNED with youth, who are the true experts. When people contact me to learn more about my clinical services, I often open by saying, "Can you tell me about your or your family's past experiences with psychiatry?Some people have good experiences, but far too many people have negative ones." I presume that people have had negative experiences with psychiatry because I've seen it so many times. Among racially minoritized folx, I see a lot of legitimate fear about being tied down and injected. Among trans folx, I see a lot of legitimate fear about transphobic providers who treat them like they are less than human. Among neurodivergent folx, I see a lot of legitimate fear about being put down or misunderstood. Though people's stories are the only evidence I need, there is also research to back up the concern that psychiatric "care" harms people. (For examples, see the following: https://lnkd.in/gG2w73sG https://lnkd.in/gWVSfZhZ https://lnkd.in/gicbVkxx) Fortunately, there are things psychiatrists can do to create a new way (see below for some ideas!). The most important thing is to not be in denial and to take responsibility for it. People often come to see us with a great deal of trepidation, and we have to put ourselves in their shoes to understand what’s at stake for them. #Psychiatrists and #MentalHealth providers, and #healthcare #providers more broadly, what are the things you can do to uncover past negative experiences and repair them. #Consumers, #survivors, and families...what do you wish your psychiatrist would do to uncover past negative experiences so they can promote healing?
