Exciting news - #Congress has negotiated a funding #deal and it includes funding Community Health Centers! Now they need to pass it - by this Friday, March 8! If you contacted your congressperson to advocate for this health center #funding, thank you! If you haven't yet had a chance, please take time today - our Wednesday Health Center Day of Action - to reach out and get this funding across the finish line! Your US House Representative and Senators need to hear that this funding bill deserves their vote; you could even remind them how many of their constituents rely on #communityhealthcenters for care. For instance, last year Community Health Centers served 31.5 million patients in urban, suburban, rural, frontier, and island communities in every state and territory. This translates to 1 in 11 Americans across over 1,400 health centers at nearly 15,000 locations. I think we can all agree, these are not insignificant numbers. Will you be part of the final push and call your members of Congress today to let them know that thousands of their constituents depend on a #bipartisan, long-term funding for health centers? If so, check out the link here: https://lnkd.in/g5_z2ZJe #valuechcs #healthcenters #fqhcs

Ensuring access to medical and behavioral health care services in rural areas is crucial, yet challenging. Check out this insightful op-ed in The Colorado Springs Gazette, LLC by Colleen Daywalt and Amy Yutzy shedding light on this ongoing issue in Colorado communities like Teller, Park, and parts of El Paso County. At CCHA, we're dedicated to increasing access to care for more than 270,000 Health First Colorado (Colorado's Medicaid program) members across eight counties. Our approach? Whole-person care. We go beyond traditional health services to address social determinants like transportation and affordable housing, recognizing their impact on health outcomes. We don't do it alone. Collaborating with local partners and providers, we're bridging gaps in care. Thanks to initiatives like the CCHA Community Incentive Program, organizations like Aspen Mine Center and Rocky Mountain Rural Health are empowering community members to navigate health care systems effectively. In El Paso, Park, and Teller counties, Ute Pass Regional Health Service District is expanding behavioral health services over 500 square miles, reducing ER visits and connecting individuals with vital support. Great meeting this morning with the American Heart Association. There are so many great opportunities in Kansas City to support and advocate for marginalized communities. I propose the following as immediate measures that healthcare and government institutions can do to help the marginalized (right now): • All health equity strategies MUST be trauma-informed and involve cultural competency as evidenced (for example) by the inclusion of “unintended consequences” forecasting during strategic planning. • Hospital systems and Federally Qualified Health Centers MUST grow closer together. Gaps in care within Kansas City’s failing patient referral system are costing us precious time and lives. A local, primary care coalition involving every institution in the chain of care is critical to success. We can do this. We all need to do this. Trust me on that one. • Case Management remains severely underfunded across the board. Whether through Community Health Workers, Cultural Health Navigators, or Social Workers, the Kansas City Community needs to continue developing a wrap-around, assisted patient experience for residents of high mortality zip codes. “You were born where? You live where? Meet your new partner who will walk with you every step of the way during your healthcare journey.” I have more than three points to make but it’s just Wednesday so we’ll stop here today. :) #healthequityisjustice #healthequityishumane #healthequityislifequality #healthequityislifeextension

What's possible when health care works more closely with the homeless response system? A lot better outcomes — for patients and systems. Explore learnings from the Healthcare and Homelessness Pilot launched by IHI and Community Solutions. "Through this pilot, a theory of change about health care’s role in ending homelessness was tested and refined. This theory includes three major pillar areas: building bi-directional awareness and commitment; establishing structures for cross-sector partnerships; and participating in a holistic system of care."
