I've been emphasizing the importance of adequate protein intake with my patients more recently, especially as they age. Recent recommendations can be as high as 1.6 grams of protein per kg of bodyweight for those 50 and up for health reasons. Surprisingly, numbers like 100 grams of protein a day are now being suggested for certain individuals, not just athletes, to maintain health. For the last 3 years, I’ve been studying how 100% juice affects our health. I’ve learnt a lot, but one thing has stood out: the gap between the data, and what people say. People: “100% juice causes weight gain and blood sugar spikes”. Research studies: 100% juice can lower blood pressure, limit inflammatory markers, and does not cause weight gain in well controlled studies. Blood sugar spikes are modest, and generally comparable to whole fruit. To be clear, 100% juice is not equivalent to whole fruit in every way, and nor is it free of all health risks. But yes, it can be a healthful beverage choice, and direct, causal evidence for any of the health risks people claim is limited. What’s driving the discrepancy? I’ve been asking myself that every day. And along the way, I have uncovered 3 core issues that I think are largely responsible. These issues are concerning and worth talking about, because they are not just specific to 100% juice. They span huge portions of nutrition & dietetics, and impact much of our current thinking in the field: 1. We’re obsessed with weight: Weight is seen as the holy grail of health. Earlier this year, people freaked out when very low quality evidence suggested children who drink 100% juice weighed just ~50 grams more than their peers. But 20+ higher quality studies, that control for confounding factors and look at health outcomes through a more holistic lens, showed no increases in weight, and a reduction in the very inflammatory markers that are mechanistically linked with higher weight and adverse health. 2. We underestimate the complexity of foods: The health effects of foods are typically judged by looking at a narrow range of key nutrients, like saturated fats and sugars. But this approach ignores the 26,000+ known biological components in foods that have health effects, and the high interconnectedness between them. When it comes to 100% juice, we mistakenly conclude that it has similar health effects vs. a sugar-sweetened beverage, simply because the sugars levels are comparable between them. 3. We have a limited understanding of bioactives: Bioactives include plant compounds that do not have a recommended daily intake but promote good health. This includes antioxidant, anti-inflammatory, prebiotic, anti-diabetic, and neuro-protective effects. 100% juice is a leading source in our diet, yet because this area is complex and our understanding is limited, we tend to ignore it.
