I don’t fit in… Being diagnosed with Parkinson’s at the age of 53 was a shock, but it eventually explained the symptoms I’d been experiencing for years. The other shock, though, was discovering how few support options were available. Parkinson’s is typically expected to affect older men and is often managed by geriatricians in clinics geared toward those over 65. Having a neurological disease at a younger age means you don’t fit in anywhere; even public health nurses are focused on under-5s and over-65s. There is a critical gap in care beyond the neurologist’s office, (when you get to see them as there are waiting lists) particularly at the community level. Neuro-rehabilitation is essential, and early intervention is key to managing long-term, progressive conditions like Parkinson’s. I sincerely hope there are plans in place to increase the number of neurologists, neuro-physiotherapists, and other specialized support staff to meet the growing demand, as Parkinson’s is now the fastest-growing neurological disease in the world. #parkinsonsawareness #neurologists #neuro #yopd #parkinsons

Medical professionals: Confidence in clinical skills”. Plain Language Summary The most common convulsive events are Febrile Seizures and these occur in 2–6% of the population. Emergency as well as recurrence management is important. We carried out this study for finding evidence for refinements in management of children with febrile seizures, including emergency and follow-up care. On the basis of the evidence findings of our study and review of existing evidence, the best possible correct and comprehensive management of Febrile seizures should include (i) First aid: Stay calm; Protect the child from injury; Turn the child onto his or her side; Do not put anything in the child’s mouth; Loosen the child’s clothing, especially around the neck (ii) Emergency seizure care and/or control is always advisable. Emergency treatment is effective with proper protocols and parents/pre-hospital teams need to be aware of the correct steps for safe treatment. If the seizure lasts for longer than 5 minutes, treatment with diazepam, lorazepam, or midazolam is required. (iii) Intermittent prophylaxis for frequently recurring febrile seizures: It is effective. Mental Health Challenges Among Physicians: Impacts of Burnout and Depression on Patient Care In recent years, the well-being of physicians has become an increasingly concerning topic due to the numerous mental health challenges these professionals face. The study Burnout, Depression, and Diminished Well-Being among Physicians, published by the New England Journal of Medicine, reveals that conditions such as acute and chronic stress, trauma, moral distress, moral injury, burnout, and depression not only affect physicians but also have direct consequences on the quality of patient care. When a physician experiences burnout, emotional exhaustion and continuous stress can lead to decreased empathy and attentiveness, undermining the doctor-patient relationship. The lack of energy and focus often associated with burnout increases the risk of medical errors, thereby compromising patient safety. Overwork and limited decision-making autonomy also contribute to professional frustration, which may result in less accurate diagnoses and a less humanized approach to care. Depression is another critical issue, particularly among physicians in training. The study reveals that during residency, depression rates increase five to six times compared to pre-residency levels, with approximately 25% to 30% of residents experiencing depressive symptoms at some point during their training. Extreme pressure and long work hours not only affect physicians' mental health but also impact the quality of care they provide. Physicians under emotional distress may struggle with making quick and decisive judgments, which are crucial in emergency situations. This scenario highlights the urgent need for interventions that promote physician well-being. Reducing working hours and providing greater psychological support can not only improve the health of these professionals but also ensure safer and higher-quality patient care. The Montreal Cognitive Assessment (MOCA) is a widely used cognitive screening tool designed to detect mild cognitive impairment (MCI) and early signs of dementia, such as Alzheimer's disease. Developed by Dr. Ziad Nasreddine in 1996, MOCA is a brief 10-minute test that evaluates various cognitive domains, including memory, attention, language, visuospatial skills, and executive functioning. The test is particularly valued for its sensitivity in detecting early cognitive changes that other assessments, like the Mini-Mental State Examination (MMSE), might miss. MOCA consists of tasks such as drawing, naming, memory recall, and verbal fluency, which provide a comprehensive picture of a person’s cognitive health. MOCA’s strengths lie in its simplicity, ease of administration, and ability to be used across different cultural and educational backgrounds with minor adjustments. However, it should not be solely used as a diagnostic tool but rather as a part of a broader clinical evaluation. Functional neurological disorder in Europe: regional differences in education and health policy: https://lnkd.in/dV8p3MiN "Of the 25 European countries for which the study had data, only five include Functional Neurological Disorder (FND) in their neurology training programs, and a mere three countries incorporate FND into their final certification exams." Surely it is similar in the USA and around the world. Considering that research suggests #FND could be as prevalent as multiple sclerosis or Parkinson's disease (https://lnkd.in/ggUQPP7U), the lack of emphasis in education and certification is deeply concerning. How can such an oversight be acceptable when so many patients are affected? How can healthcare systems expect to effectively diagnose and treat patients when future neurologists are not even properly trained on this disorder? The absence of #FND in medical training not only hampers awareness but also risks the quality of care for potentially millions of patients. Incorporating #FND into the curriculum and increasing awareness among neurologists is critical. Without proper training, future neurologists may miss or misdiagnose #FND, leaving patients without adequate care. By improving education on #FND, we not only foster earlier recognition and better management but also reduce the stigma surrounding the disorder. #FND deserves a place in neurology (and other specialties) training, and expanding its presence in the curriculum is an urgent step toward better patient outcomes.

When I began my journey in the field of Functional Neurological Disorder in 2016, I remember devoting considerable effort orienting myself to the field by identifying the leading experts in adult FND, their institutions and the nature of their research. Today, eight years later I am proud to share this collaborative effort with my colleagues and friends that extends this work into pediatric FND, an even more underserved population.
