Arthrofibrosis is a condition where too much scar tissue forms in and around the knee after a total knee replacement (TKR), causing stiffness and pain: 1. Inflammation and Healing Response: After knee surgery, the body reacts to the trauma by sending signals to heal the area. This involves releasing chemicals called cytokines that help form a blood clot and attract immune cells (white blood cells) to the site. 2. Excessive Scar Tissue Formation: - Chemicals like PDGF and TGF-β promote the growth of cells that produce collagen and other materials, which build up and create thick scar tissue. - VEGF is another chemical that increases inflammation and scar tissue by promoting new blood vessel growth in the knee. 3. Lack of daily high intensity stretching: With home exercises only being 10% of the torque of a physiotherapist’s manual therapy the knee isn't moved enough after surgery. So tissues become stiff, and the excessive production of collagen leads to continuation of scar tissue build up. This causes a loss of ROM, impacting simple daily activities. March 12, 2024 was World Glaucoma Day under the theme 'UNITING FOR A GLAUCOMA-FREE WORLD'. Glaucoma: a disease of the optic nerve of the eye. There are many types. Glaucoma is a chronic, progressive degeneration of the optic nerve, resulting in lost of the peripheral vision, followed by lost of central vision. It is the leading cause of irreversible blindness, It is the second leading cause of blindness. Risks: Aging, black race, family history of Glaucoma, elevated eye pressure. It cannot be cured but can be controlled , It is mostly diagnosed on routine eye examination. Treatment: lowering eye pressure with eyedrops and or surgery, regular follow-up appointments with eye doctor. Recommendations: Regular eye examination from age 35. Shoulder replacement is a surgical procedure in which all or part of the glenohumeral joint is replaced by a prosthetic implant. Such joint replacement surgery generally is conducted to relieve arthritis pain or fix severe physical joint damage. Shoulder replacement surgery is an option for treatment of severe arthritis of the shoulder joint. Arthritis is a condition that affects the cartilage of the joints. As the cartilage lining wears away, the protective lining between the bones is lost. When this happens, painful bone-on-bone arthritis develops. 3-column fractures in the ankylosed spine are extremely unstable, and can pose a risk of neurological injury if they become displaced. These injuries can also be subtle on imaging, and may occur after reasonably low-energy trauma. When treating trauma patients with AS/DISH it is critical that you maintain a high index of suspicion for unstable fracture. This lovely 55-yo-female sustained a ground level fall with an extension-type mechanism, and subsequently presented with excruciating, axial pain in the upper lumbar spine. Imaging demonstrated 3-column injury at L1-2, and she was taken for operative stabilization. One year out from surgery, the patient is doing great, with minimal pain, and return to pre-injury level of function. (shared with patient permission) #spinesurgery #orthopaedics #trauma #patientcare

Honoured to have been a guest speaker at the KSES 31st Annual Congress in Seoul, Korea, thanks to my friend JC Yoo, President of KSES. My talk focused on the role of Reverse Shoulder Replacements in under 55 year olds. While it is not common to replace the shoulder in this age group, there is early evidence from the Australian Joint Replacement Registry (NJRR) that suggests Reverse Total Shoulder Replacement and Stemless Anatomical Total Shoulder Replacements may be a better option for younger patients. These alternatives may have a lower revision rate than the classical Hemiarthroplasty or Anatomical Total Shoulder Replacements, which have been traditionally preferred. High activity levels in younger patients can lead to faster wear and tear, resulting in the need for further surgery to revise the joint replacement at some stage. #shoulderreplacements #KSESannualcongress #reverseshoulder #stemlessshoulder #jointreplacement #healthcare

#IONM used to help better understand safety guidelines for foot surgery using tourniquets... results below. Tourniquets used to prevent bleeding can cause ischemic injury. Like most everything else, it's dose-dependent. So how much pressure and for how long seem like reasonable questions. SSEP and MEPs were used to help determine some guidelines. Here are the results: "Group 1 (LOP + 50 mmHg) exhibited a 50% decrease in the femoral nerve MEP value at an average of 47 min, whereas group 2 (LOP + 100 mmHg) demonstrated a 50% decrease at an average of 34 min (p < 0.001). The duration of a 100% decline in MEP for all nerves was significantly longer in Group 1 than in Group 2 (p = 0.007; p < 0.01). The total recovery time of MEP for all nerves was significantly shorter in Group 1 than in Group 2 (p = 0.007; p < 0.01)" More pressure = shorter to changes in the signals and longer to recover. No big surprise there. The next set of questions -- better answered with a larger group -- would help answer questions like injury scope, transient vs pertinent, identify intraoperative criteria with the most postoperative risk, and decide if monitoring all these cases would benefit patients vs coming up with tighter set guidelines. #neuromonitoring #CNIM #footsurgery #orthopedicsurgery Yıldırım, A.
