It is exciting to see the implementation of the Mobile Integrated Healthcare initiative at Brattleboro Memorial Hospital (BMH) unfold in rural Vermont! This new program aims to provide better support and care to patients undergoing total joint replacement (TJR) surgeries. Given the transportation challenges, restrictions in health insurance coverage, and staff demands, patients often have pre-operative and post-operative needs that are inadequately met. By partnering with Rescue Inc., a local paramedicine service, this program trains existing paramedics and technicians to provide consultations and evaluations either in-person or via telemedicine, available 7 days a week, 365 days a year, and 24 hours a day. This initiative results from the supplemental award we were granted by the Federal Office of Rural Health Policy (FORHP) to support organizations across the Northern Border Region with focused one-year healthcare programming. In collaboration with the Northern Border Regional Commission (NBRC), this funding was awarded to seven organizations in Maine, New Hampshire, New York, and Vermont to support innovative programs designed to enhance healthcare services to rural residents in all four states. This makes it less accessible for many individuals, especially without insurance coverage. 2. Limited Availability Appointments with private doctors can be difficult to secure due to high demand. Furthermore, their availability may be restricted to certain hours, unlike public healthcare where doctors are often available round-the-clock. 3. Inconsistent Care You may not always see the same doctor, especially in larger private practices or clinics. This lack of continuity can lead to inconsistent care and treatment plans, making it harder to manage long-term health concerns. 4. Profit Motive Some private doctors prioritize financial gain over patient well-being. Every patient deserves access to health care, but how do we ensure it reaches them? While working in almost every health care industry sector, I have seen the impact of #340B in helping eliminate #healthcaredisparities around the nation. This recent report from the #AHA affirms that when coupled with partnerships between community and #specialitypharmacies, 340B enabled providers to reach more patients with a wider range of care, especially in communities with a “ lack of insurance, food insecurity, high unemployment, and severe housing problems.” This is #WhyIMarch.

Overall, a recent study suggests that hospital privatisation "may reduce costs" but "at the expense of quality of care." Key findings reveal that increases in privatisation often result in worse quality of care, with no evidence of unequivocally positive health outcomes. Hospitals transitioning from public to private ownership tend to prioritize higher profits by cutting staff levels and reducing coverage for patients with limited health insurance. Additionally, privatisation is linked to fewer cleaning staff per patient and higher rates of patient infections. Some studies even indicate that higher levels of hospital privatisation lead to increased avoidable deaths. However, in certain cases like Croatia, privatisation has improved patient access through precise appointments and innovative care delivery methods. May 19th - 24th 2024, marks the 50th anniversary of EMS Week, a time to celebrate the exceptional care provided by highly trained EMS professionals. However, it's also time to acknowledge our outdated EMS reimbursement model that fails to recognize their value. Our current model ties payment solely to transport, ignoring the fact that EMS providers are not just suppliers of services but rather healthcare providers.
