Hospital care coordination is a vital, behind-the-scenes responsibility that often goes unrecognized. Would I recommend pursuing a career as a hospital RN Case Manager or Clinical Social Worker? No, not with the current operations of hospitals. In a nationwide survey of acute care case managers, 92% reported that they do not have sufficient resources to appropriately inform patients about post-acute care options. I spoke with an RN case manager at a 600+ bed hospital. During her weekend rotation, she was responsible for 87 patients and did not have time to leave her office. 16 patients were discharged over the weekend with post-acute care plans. Her primary focus was conducting InterQual screenings to anticipate potential discharges to skilled nursing facilities. I wonder how many of the discharged patients were readmitted to the hospital. How many of the non-discharged patients stayed an extra night due to unanswered questions? She’s actively seeking a new profession. If you want to improve case managers' experience while lowering readmission rates and reducing lengths of stay, DM me or go to GuidingPatients, Inc.. #Healthcare #PatientExperience #Hospitals

Nurse Practitioners (NPs) play a vital role in rural healthcare, often serving as the primary healthcare providers in areas with limited access to medical services. In rural communities, NPs offer essential services—everything from preventive care and chronic disease management to mental health support and urgent care. This accessibility is crucial for residents who might otherwise need to travel hours to the nearest hospital or clinic. With their holistic, patient-centered approach, NPs in rural areas also address social factors that impact health, working closely with patients to navigate challenges like transportation, food security, and mental health. Their role isn’t just medical—it’s about community connection and resilience. At the Alabama AHEC Network, we support programs and initiatives that help nurse practitioners thrive in rural areas, ensuring all Alabamians have access to the compassionate, high-quality care they deserve. Join us in recognizing and celebrating NPs for their dedication to rural healthcare! #NursePractitioners #RuralHealthcare #HealthcareHeroes #AlabamaAHEC

Are you a current or former social work or nurse case management professional in a healthcare setting? I am asking for your assistance in completing and sharing this research study with your team of social workers and nurses. Participation would involve completing an online survey that takes approximately 15 minutes. The survey aims to identify factors that enhance effective case management and improve patient outcomes. All responses will be kept confidential, and the insights gained from this research could contribute to advancing case management practices. This expands eligibility for appointment to county boards of health and health services advisory boards to include nurse practitioners and physician assistants; expands to residents in the county or residents of a contiguous county who have a background in a health-related field and perform substantial work in the county associated with their health-related background.

It's hard to believe how much has changed in the time I've been working in healthcare, especially in primary care here in BC. To be completely honest, it's been a bit disheartening to witness the growing chaos within the system 💔 It's almost surreal to think that things are actually getting worse instead of better. Every day, it feels like we're inundated with referrals from sources like the nurses hotline and 8-1-1, both of which are government-run programs that are supposed to alleviate pressure on the healthcare system. Instead, they seem to be adding to the burden, sending us 3-4 referrals daily 🙏 It's a difficult position to be in because, on one hand, we want to help as many people as we can, but on the other hand, it's disconcerting to see the government inadvertently contributing to the strain on healthcare resources. If you or someone you know is struggling to find a family doctor or nurse practitioner (or just primary care), please don't hesitate to reach out to us. Myself or Care2Talk | Connectivity & Primary Care may not have all the solutions, but will do whatever we can to assist you in navigating through these challenging times. #primarycare #familydoctor #healthcare #solutionforward #acceptingnewpatients
