This is not surprising, nor is it new. Most computer users are never really trained in the importance of security regarding the data that they handle. Some, unfortunately, just don't care. There are many ways to systemically prevent these events, but it means investment in hardware, software, and more training. And most companies just don't care. BTW ... we need to call this what it really is.
