With nearly 3 decades of wisdom and experience under his belt, Dr. Victor Sierpina from UTMB® emphasizes the importance of long-term doctor/patient relationships. His word of advice: continuity of care improves health outcomes, reducing unnecessary ER visits. At PeopleOne Health, we couldn't agree more. We're modern, connected, and people-focused. We built our care model to foster meaningful connections between our care teams and members to keep people healthier. https://bit.ly/3UIoaBI

On Christmas Eve, I'm sharing an essay in BMJ about compassion in health care. I cheer author Al Aynsley-Green for everything he says about the need for demonstrations of compassion in health care, including how diagnosis or unexpected news is shared and tender end-of-life care. He gives examples of how compassion can be taught and measured. Patients have been saying how much compassion matters for them for a very long time. If only people would listen. https://lnkd.in/g3E5puqT

End-of-life care is not a #sideline issue but a core #component of #compassionate, patient-centered #healthcare. By improving #palliative #care #literacy among #medical #professionals, we can ensure that #patients receive the #support and #respect they deserve in their final #days. This extends beyond #medical #care, and refers to the #ability to #communicate with #empathy and #clarity, guiding patients and their families through some of the most challenging #decisions they will ever face. As #providers, our #goal should be to create a healthcare #environment where #conversations about #death are not avoided but instead are #approached with #sensitivity and #competence. This approach can improve #outcomes for patients and families, #reduce the #emotional #burden on healthcare #providers, and ultimately reflect a more #holistic and #personalized #approach to #medical care. Death is an #inevitable part of #life, and our healthcare system must be better equipped to handle it. By #fostering open, #honest, and compassionate #discussions about end-of-life care, we can provide patients with more #dignity throughout their #experience. Or, at the very least, we can meet patients at the #bedside with #tact and #grace when supporting their wishes and needs near the end of their lives.

ADTs are simple but powerful alerts that can streamline care coordination, reduce costly readmissions, and, most importantly, improve patient quality and outcomes. Learn more about the CA ADT Network below!
Our goal is to help get more ADTs to the proper care teams and reduce the burden for organizations to meet their DxF requirements in alignment with California public health, CalAIM, and health equity priorities. As a palliative care chaplain, I have witnessed firsthand the profound impact that spiritual care can have on patients and their families during some of life’s most challenging moments. Our role extends beyond traditional spiritual support; we are integral members of the interdisciplinary healthcare team, bringing a unique perspective to patient care. What can a palliative care chaplain bring to the table? -Understanding Meaning and Values: We help patients articulate what is meaningful to them—whether it’s family, faith, or personal passions. By exploring these values, we empower patients to make medical decisions that resonate with their life story and goals. -Facilitating Difficult Conversations: Navigating the complexities of serious illness often requires tough discussions. Chaplains are trained to engage patients and families in conversations about end-of-life preferences, fostering understanding and reducing fear around medical choices. -Supporting Emotional and Spiritual Well-being: We address not only the spiritual but also the emotional needs of patients. By providing a safe space for reflection and connection, we help alleviate feelings of isolation and despair, enabling patients to focus on what truly matters to them. -Promoting Holistic Care: Our presence on the team ensures that spiritual health is prioritized alongside physical and emotional well-being. This holistic approach enhances the quality of care and supports better outcomes for patients. By integrating palliative care chaplaincy into the healthcare team, we can profoundly affect the patient experience, ensuring that care is not only compassionate but also aligned with what matters most to each individual. Together, we can champion a future where every patient feels heard, valued, and supported on their journey. #PalliativeCare #Chaplaincy #PatientCenteredCare #HealthcareTeam #SpiritualCare #HolisticHealth #SpiritualCareWeek
