Usually proximal or hilar cholangiocarcinoma presents as localized circumferential wall thickening, deep to the PV and HA, so very difficult for EUS tissue aquisition! Sampling of nearby porta hepatitis LN is usually possible and will be an indirect evidence of hilar malignancy if proved metastatic LN. Spyglass is not available all the times!!!
