The importance of understanding both anatomy and fluorscopy are fundamental for any higher level spine or minimally invasive procedure. I stress the importance of this to all my rotating residents and fellows at University of Rochester Medical Center. Below is a case of a patient with chronic back pain with scoliosis , rotational deformity, transitional anatomy, and a L5-S1 anterolisthesis. L3-S1 vertebral levels were treated with the #intraceptprocedure by Boston Scientific. The initial left most image is a lumbar spine AP. The middle image is a line drawn for the squared off superior endplates of L5, L4, and L3 levels with the cannula lined up for a left L3 transpedicular trajectory. A line is drawn with a sponge clamp to assit the Rad tech when transitioning to lateral for appropriate wig-wag view. Whats amazing is the third view which is the OBLIQUE transpedicular view for a left L3 access. I am thrilled to share that our latest international research project, titled "Prevalence, risk factors, natural history, and prognostic significance of Modic changes in the cervical spine: a comprehensive systematic review and meta-analysis of 12,754 participants", has been published in the prestigious Neurosurgical Review journal (Q1, Impact Factor: 2.5). Many thanks to Ahmadreza Nezameslami, Samuel B. Kankam. MD, MPH, Mobin Mohamadi, Aynaz Mohammadi, ALIREZA KHOSHNEVISAN, Joshua Chalif, Yi Lu and John H. Chi, MD MPH This systematic review and meta-analysis sheds light on an underexplored area in spinal research—Modic changes in the cervical spine. Our study, involving over 12,000 participants, dives deep into their prevalence, risk factors, natural history, and clinical significance, particularly in surgical outcomes. Cervical modic changes are present in 26% of individuals, with type 2 being the most common. While modic changes did not significantly impact neck or radicular pain, they were associated with prolonged recovery periods post-surgery, highlighting the need for tailored postoperative care.
