A. Within 5 days B. Within 50 days C. Within 5 months D. Within 5 years The “textbook” answer is B. Within 50 days because, technically, that is the best “biologic“ time to perform the surgery, if there is a true full thickness, complete, ACL tear where the ends are not touching, and there are no intact fibers. Also where the ACL has not “scarred“ to the PCL. The reality is that as long as there is a stump on the tibia/shinbone that we can anchor a couple of passes through with a suture passer, usually about at least 1 cm, then, no matter how old the tear, it is very likely the BEAR surgery can be a success. So, there is no actual definitive timeframe as to how far after an ACL tear BEAR surgery can be performed. Today’s Poll Question: How much does a BEAR implant cost? What is the posterolateral corner? 🤔 A group of ligaments and structures that stabilise the outer knee and connect the femur (thigh bone) to the lower leg bones, the tibia (shin bone), and the fibula. It's a complex arrangement that provides critical stability to the knee. 🔍 How is it injured? The posterolateral corner of the knee can be injured in a high-energy accident like a car crash or motorbike accident. It can also happen in a fall or sports injury where the knee is forced to hyperextend. These injuries make your knee feel painful, swollen, and unstable. 🔍 How is it treated? Mild posterolateral corner injuries may be treated with bracing. Today's Collab topic : Knock Knees Knock knee is a condition in which the knees bend inward and touch or "knock" against one another, even when a person is standing with their ankles apart. This places excessive force on the outer side of the knee, which can cause pain and damage over time. These symptoms may include: knee or hip pain foot or ankle pain feet not touching while standing with knees together stiff or sore joints a limp while walking reduced range of motion in hips difficulty walking or running knee instability. Treatment If knock knee is caused by an underlying disease or infection, that condition will be addressed before any orthopedic correction begins. Treatment for mild cases of knock knee in children or adolescents may include braces to help bones grow in the correct position. If a gradual correction does not occur, surgery may be recommended. Do you ever wonder what’s the statistics on patient compliance with their weight bearing precautions? Let me tell you. According to a research study, it is found that about 88% or 22 of the 25 people with ankle or foot fractures or surgery that has non weight bearing precautions, had in one way or the other, broken their precaution. It is even after instructions or education with emphasis on compliance. Thus increasing re-injury. We know that a person with recent ankle or foot injury or just right after surgery, has little to no confidence. Let alone the big insecurity when walking. And taking the patient’s side, did you even think how hard it is for them to comply? Well, there’s always a device solution. But, we have to ask. Is it the right one for this particular patient? There is a better solution for repairing your Achilles injury. Use tunnels and HS-Fiber suture tape. The antiquated speedbridge kit only adds extra cost and extra pain for a patient. Four anchors embedded in your heel are unnecessary in today's world. Tensor Surgical eliminates the need for plastic/biocomposite(goop) in this procedure, provides maximum haemopoiesis for healing, and can now be performed with a reproducible, minimally invasive technique. Transosseous surgery, the gold standard for benchmarked outcomes for a century, has been making a comeback in the orthopedic world. Christopher Lotufo, DPM, FACFAS, has adopted the technique because of many benefits, including less pain, haemopoiesis needed to heal the tendon, increased footprint coverage correlated with improved tendon healing, along with fewer complications and revision scenarios compared to suture anchor-based repair. Because this technique obviates the need for anchors, significant cost savings of over a thousand dollars are truly on par with value-based care. I honestly can't understand why all doctors don't use this technique. A two-thousand-dollar “Kit” over tunneling and HS-Fiber sture tape does not add value for all parties. It only adds value TO THE ONE MAKING THE KIT.
