I am thrilled to have attended today's webinar, "Advances in Pancreatic SBRT with the cyberknife system," by AEx. The following were critical points noted; >Stereotactic body radiotherapy (SBRT) for pancreatic cancer is an advanced radiotherapy technique that delivers high-dose fractions of radiation precisely to the tumor while minimizing exposure to surrounding healthy tissues. >This method is particularly advantageous given the pancreas' proximity to critical structures such as the stomach, intestines, and liver. >SBRT is typically used in cases where the pancreatic cancer is locally advanced or borderline resectable. It is also an option for patients who cannot undergo surgery due to other health conditions. >The treatment involves delivering a few high-dose fractions of radiation over a short period, usually five sessions or fewer, compared to traditional radiotherapy, which may require many weeks of daily treatments. One of the main benefits of SBRT is its precision, which reduces the risk of damage to nearby organs and tissues. This precision is achieved through advanced imaging techniques and sophisticated technology to guide the radiation beams. >However, SBRT is not without risks. What Are the Specific Benefits to Breast Cancer Patients? Breast cancer patients who have had skin and blood vessel damage, as a result of radiotherapy or surgeries, including mastectomy and breast reconstruction, can benefit significantly from HBOT treatments. Blood vessels can become narrowed and scarred in areas that have been irradiated. This can cause tissue damage months or even years after radiation therapy. Skin-sparing and nipple-sparing mastectomy procedures can also lead to skin loss or mastectomy flap necrosis. HBOT can be a transformative treatment in these types of circumstances. Pancreatic Adenocarcinoma: Long-Term Outcomes of Adjuvant Therapy in the ESPAC4 Phase III Trial out on JCO The ESPAC4 trial analyzed long-term survival outcomes of patients with resected #PancreaticCancer treated with either gemcitabine or a combination of gemcitabine plus capecitabine (GemCap). With a median follow-up of 104 months, GemCap showed a significant overall survival (OS) benefit over gemcitabine alone, with median OS of 31.6 months versus 28.4 months, respectively (HR 0.83, p=0.031). Particularly, R0 patients (those with no residual tumor after surgery) had a notable survival advantage with GemCap, achieving a median OS of 49.9 months compared to 32.2 months with gemcitabine (HR 0.63, p=0.002). Additionally, lymph node-negative patients on GemCap had improved 5-year OS rates. The study suggests that GemCap is an effective adjuvant therapy, especially for patients not eligible for the more intensive mFOLFIRINOX regimen. After a multidisciplinary team review, Justine underwent a mastectomy, followed by chemotherapy and radiotherapy. Dr Dusseldorp detailed the complex reconstruction process that followed, including a bilateral DIEP flap procedure. This is a modern breast reconstruction technique that uses skin and fat from the lower abdomen while sparing the abdominal muscles, resulting in reduced abdominal wall injury, less weakness, and a quicker recovery. Justine also chose sensation reconstruction, a technique aimed at restoring feeling in her breasts. This added 30 minutes to her eight-hour surgery but resulted in a significant improvement in sensation, helping her feel more connected to her body. “They just feel part of me now,” she said. As evidence of the sensory benefits continues to grow, more plastic surgeons are likely to incorporate sensation reconstruction into their DIEP flap procedures. Patient advocates are also raising awareness about the issue of numbness after mastectomy, urging a shift towards incorporating sensory preservation in standard reconstruction practices. With October being Breast Cancer Awareness Month, it is imperative to share patient experiences, as the future success of this technique relies on continued advocacy and the widespread sharing of successful outcomes.
