To mitigate these risks, early identification and management of GDM is critical. Treatment for GDM aims to maintain normal blood sugar levels and typically includes glucose monitoring, adjustments to diet, lifestyle changes, moderate physical activity, and medication as necessary. Insulin administration is generally the preferred choice for pharmacotherapy, but oral drugs such as metformin or glyburide may also be appropriate. Compared to glyburide, which has been linked to higher birth weight, neonatal hypoglycemia, raising the risk of shoulder dystocia, and necessitating a caesarean delivery, metformin is seen to be a safe first-line therapy. It should be emphasized that some expecting mothers choose complementary and alternative therapies, such as traditional herbal supplements and treatments. DAY 3 Knowing drugs ~Metformin💊 - Drug Class: Biguanide - Mechanism of Action: Decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. - Uses: Metformin is primarily used to treat type 2 diabetes, particularly in overweight individuals, when dietary management and exercise alone have not been enough to control blood glucose levels. It may also be used in polycystic ovary syndrome (PCOS) to improve insulin sensitivity and regulate menstrual cycles. - Dosage: The typical starting dose is 500 mg twice a day or 850 mg once daily with meals, gradually increasing to a maximum of 2,000–2,500 mg per day in divided doses. - Administration: Metformin is taken orally in the form of tablets or extended-release tablets. - Side Effects: Common side effects include gastrointestinal issues like diarrhea, nausea, and abdominal pain. These side effects often subside over time. In rare cases, it can cause lactic acidosis, a serious condition. - Precautions: Metformin should be used cautiously in patients with impaired kidney function due to the risk of lactic acidosis. #kidneydisease #drugsafety #nephrology https://lnkd.in/gsHUgnq8 Abstract People living with chronic kidney disease (CKD) often experience multimorbidity and require polypharmacy. #Kidney #dysfunction can also alter the #pharmacokinetics and pharmacodynamics of #medications, which can modify their risks and benefits; the extent of these changes is not well understood for all situations or medications. The principle of drug stewardship is aimed at maximizing medication safety and effectiveness in a population of patients through a variety of processes including medication reconciliation, medication selection, dose adjustment, monitoring for effectiveness and safety, and discontinuation (deprescribing) when no longer necessary. This Review is aimed at serving as a resource for achieving optimal drug stewardship for patients with CKD. We describe special considerations for medication use during pregnancy and lactation, during acute illness and in patients with cancer, as well as guidance for the responsible use of over-the-counter drugs, herbal remedies, supplements and sick-day rules. We also highlight inequities in medication access worldwide and suggest policies to improve access to quality and essential medications for all persons with CKD. Further strategies to promote drug stewardship include patient education and engagement, the use of digital health tools, shared decision-making and collaboration within interdisciplinary teams. Throughout, we position the person with CKD at the centre of all drug stewardship efforts.

Delving Deeper into Type 2 #Diabetes Treatment Options: Beyond the Basics Understanding the Mechanisms: * #GLP-1 Receptor Agonists: These injectable drugs target Glucagon-like Peptide-1 (GLP-1), a natural gut hormone. When GLP-1 receptors are activated, they: * Stimulate #insulin secretion from the pancreas, lowering blood sugar after meals. * Suppress glucagon release, a hormone that raises blood sugar. * Delay stomach emptying, leading to feelings of fullness and potentially promoting weight loss. * #SGLT2Inhibitors: These medications act on the Sodium-Glucose Cotransporter 2 (SGLT2) protein in the kidneys. Normally, SGLT2 reabsorbs sugar from the urine back into the bloodstream. SGLT2 inhibitors block this process, causing excess sugar to be excreted in the urine, effectively lowering blood sugar levels. Additionally, they may: * Improve heart health by reducing blood pressure and inflammation. * Benefit kidney function by lowering the workload on the kidneys. * Basal Insulin: Our bodies naturally produce basal insulin, a steady background level throughout the day.
