FAKE PILLS! HOW CAN YOU TELL YOUR PROVIDERS? Recently, we tested a urine specimen from a patient that was on #oxycodone for #chronicpain. The #confirmatorydrugtesting came positive for fentanyl and its metabolite nor fentanyl. The prescribing clinician was a primary care physician. He called the lab and said, “the patient denies consuming fentanyl and the patient is 65 years old female that has been my patient for several years and I trust her”. We looked at #chromatographypeaks on the #LCMS and everything showed that there was no mistake. When we contacted the prescribing physician and told him that we think there is no mistake and the patient most likely consumed fentanyl, he was not happy with our answer. There is a possibility of laboratory error with urine drug testing using confirmatory method. However, we think there are other explanations for this event. Recent reports from CDC (1) and DEA (2) indicate that fake pills consumption is growing in the United Sates at a fast pace. We think it is possible that the patient used a fake pill that was laced with fentanyl. When we searched to see if there is any scientific report discussing the effects of fake pills on laboratory results of drug testing, unfortunately, NONE was found. A lesson from Toxicology Drug Testing!
