“Mr Lee, you have this XXX diagnosis and this suit of services is what you need. It’s our standard care package that we offered to patients with this XXX diagnosis” Mr Lee: “Oh really?” 🙄 💡 It’s more important to know the person who has the disease and not the disease whom the person has. What is person-centred care (PCC) and how do we implement in reality? PCC is treating the patient as individual and equal care partner in the care journey. It’s personalized, coordinated and enabling. This needs to be embedded in all levels of healthcare delivery. I would like to share this post from the perspective of dementia. Person-Centred Care in dementia places a high value on the person living with dementia and their care partners or caregiver. It regards people as unique individuals deserving of understanding and respect, with opportunities to continually engage in positive interactions and experience well-being in their lives. 👨💼 The practice of PCC begins by looking at the individual as a person 👱♀️ whose dementia forms only part of his total identity. The larger part of this individual continues to be the possession of his or her own uniqueness, the retention of the need for self-recognition and identity as well as the preservation of their ability to act and respond to the environment that surrounds them. 😉 Person-Centred Care also acknowledges that the care partner, whether family member, an employed helper at home or a professional care partner, needs to be valued and supported. They have to be empowered to make decisions and how care plans should be carried out. 🏢 An organisation that follows Person-Centred Care model will demonstrate this philosophy through their vision and mission statements as well as through policies regarding hiring, training, supervising and recognising professional care partners and their contributions. There will be resources to build an environment supporting Person-Centred Care practices and the staffs understand this value system and feel responsible for building and sustaining this approach. #dementia #caregiver #personcentredcare #ccpn #danlinluo

He is a little tidbit for you 💜💜 #dementiacare #daycationforseniors #alzheimerscare #seniorcare Careblazer, There is something I witness a lot when working with families. It's when we ask questions just to see if the person with dementia knows the answer. It's for our own curiosity not because we need to know. We already know the answer, but we want to see if the person with dementia knows the answer. These questions may sound like: "What day is today?" "Do you know who this is?" "What did I tell you yesterday about dinner?" Asking questions that you already know the answer to just to see if your loved one knows the answer is called "testing." Testing can make the person with dementia feel defensive, upset, anxious, or sad. It also puts strain on the relationship they have with you. You will have plenty of experiences and situations throughout your caregiving journey that will reveal how their memory is doing. You don't need to create more situations out of curiosity. Rather than testing, consider telling. Telling the person with dementia the information you want them to have rather than testing to see if they know the answer is the preferred approach if you care about the relationship you have with them. 💖, Dr. Natali
