The addition of needed medications per patient increases the likelihood of negative drug interaction side effects, which can lead to unnecessary falls for patients. Having an integrated behavioral health program can help skilled nursing centers recognize these patients who are at risk. Our clinicians collaborate with the facility's multidisciplinary team and provide evaluations to assist in diagnosis and treatment recommendations to improve patient care and functioning. Determinations are then made on who will benefit from non-pharmacological psychological treatment, or who may need further medication management - all while assuring your compliance with current state regulations. https://lnkd.in/ejyK9P_n #SkilledNursing #BehavioralHealth #PatientCare #MedicationManagement #Healthcare #MentalHealth #TreatmentRecommendations #IntegratedCare #ClinicalCollaboration #Compliance #che

Why Are Only 14% of Those Needing it, Receiving Palliative Care? It's a startling statistic! This gap is due to several factors rooted in the history and evolution of palliative care. Originally, palliative care was born from hospice care, which was intended for patients in the final stages of life, typically the last three to six months. Now, palliative care is recommended for anyone diagnosed with a serious illness from the time of diagnosis. This expansion has dramatically increased the number of patients who could benefit from palliative care. However, there’s a significant mismatch between the number of patients in need and the availability of trained providers. This is why it's crucial to train as many healthcare professionals as possible—beyond specialist palliative care physicians—to deliver primary palliative care. Whether it’s naturopathic oncologists, cardiologists, nurses, or social workers, all members of the healthcare team must be equipped to provide the best possible palliative care to patients, their families, and caregivers. We must bridge this gap to ensure every patient receives the comprehensive care they deserve. I talk more about this in the following video: https://buff.ly/3AURZHH #PalliativeCare #HealthcareTraining #SeriousIllness #PatientCare #OncologyCare #QualityOfLife #MedicalEducation #InterdisciplinaryCare

The Importance of Family Medicine in Reducing Healthcare Costs Recent studies have shown that a robust primary care system can significantly lower healthcare costs. Family medicine plays a crucial role in this by providing comprehensive, continuous care that focuses on prevention and early intervention. When patients have access to quality primary care, they are more likely to receive timely treatments, reducing the need for expensive emergency services and hospitalizations. Family physicians build long-term relationships with their patients, allowing them to understand their health histories and individual needs better. This personalized approach leads to better health outcomes and ultimately saves money for both patients and the healthcare system. Let’s advocate for stronger support for family medicine and primary care initiatives. Together, we can make a difference! #familymedicine #primarycare #aafp

Today is National #HealthcareDecisionsDay, a day dedicated to educating and empowering the public and healthcare providers about the importance of advance care planning. In the absence of an advanced directive, medical personnel rely on family members to determine what a patient's wishes might be. Family members also often struggle to make these difficult decisions. However, many of these difficult decisions can be avoided with advanced planning. 📹 Erica Schockett, MD, System Division Chief for Palliative Care and Geriatric Medicine at Inova, shares why your decision matters. "We want to honor what matters most to you. So it's important to put your wishes in writing," she said. Watch! Learn more: https://lnkd.in/gQC6HbSf #advancecareplanning #advancedirective
