Australia's first cohort of candidate Aged Care Onsite Pharmacist (ACOP) and Medication Management Review (MMR) pharmacists have commenced the Pharmacy Care Academy's credentialing. I extend my gratitude to Regis Aged Care Sandgate for hosting our work-integrated-learning. Coal-face exposure is the only way to effectively prepare pharmacists for the practicalities and complexities of working within the residential care setting. #industry_preparedness #career_opportunities #medication_safety_champions

Something interesting about working in Long Term Care Pharmacy is knowing when certain laws apply to SNFs/LTCs/Hospice (skilled nursing, long term care) vs ALFs (Assisted Living Facilities). One key feature is controlled & non-controlled substances prescribing rules. I created this Arizona Controlled Substance chart back In February to help with my own understanding of Controlled Substance rules in regards to these facilities that changes based on their designation. Feel free to share with those that are involved in providing any sort of care for these patients as this knowledge is valuable. #longtermcare #hospice #hospicecare #hospicenurse #hospicelife #longtermcareawarenessmonth #azbop #azpharmacy #azpharmacist #azpharmacyassociation #azpharmacypractice #pharmacist #pharmacists #pharmacytechnician #pharmacytechnician #pharmacyeducation #pharmacylife #nurselife #doctorlife #controlled #narcotics #c2 #doctorsofarizona #nursesofarizona #nursesoflinkedin #doctorsoflinkedin Ensign Services Sandstone Healthcare Group AZ Arizona State Board of Pharmacy Arizona Department of Health Services Bureau of Long Term Care

Reminder: A complete reconciled medication list that was discussed with resident and provider ensures safe medication administration. ESPECIALLY after a hospital stay. Nurses: 4- steps-- Verify (collect a current medication list) Clarify (make sure the medications and doses are appropriate) Reconcile (compare new medications with the list and document changes in the orders) Transmit (communicate the updated and verified list to the appropriate caregivers, like pharmacy) As defined by the JCAHO, medication reconciliation is "the process of comparing a patient's medication orders to all of the medications that the patient has been taking." #assistedliving #supportiveliving #eldercare #dotherightthing #nurse #pharmacist #Physician

Pharmacy First – how’s it going? We caught up with Amanda Kelly, Community Pharmacy Derbyshire committee member and Locally Commissioned Contracts Technician for Rowlands Pharmacy in Bolsover to find out how Pharmacy First is working for the team there. Over to you Amanda! How many PF consultations have you carried out? As of 3rd April we’d carried out 38 consultations. What are the most common of the 7 conditions you are seeing and treating? UTI, earache and sore throat. What is the split between self-referral/walk in and GP referral? The majority so far are patient self-referrals. We are getting GP referrals just usually not through PharmOutcomes. Patients will usually come into the pharmacy and have been to the GP and are unable to get a GP appointment, so the receptionist will have signposted them down to us here in the pharmacy. What has the response to the PF service amongst your customers? Patients are very grateful especially with the lack of GP appointments. Not having to wait long for this service and receiving treatment straight away is something they are definitely pleased about. AMANDA'S TOP TIPS ✅ Advertising - we have a great display to advertise and we put all advertising cards into prescription bags. We post on our social media pages too and have newly extended our Saturday hours to help promote this service. ✅ Engagement - we engage directly with our patients over the counter before we make a sale on certain products as they can often be eligible for this service. #communitypharmacy #pharmacyfirst #pharmacyteams #pharmacytechnicians

People have been asking why I decided to nominate to continue on the Pharmacy Council of NSW, so let me share my why. Let’s be honest… pharmacists are doing it tough. You don’t need to be a community pharmacist or a pharmacy owner to see the challenges we’re all navigating. The expanding scope of practice? Amazing for the profession but it also means rising pressures and expectations from the community. And when a complaint or notification comes your way? That’s a stress no one signs up for lightly. The Pharmacy Council of NSW plays a key role in managing complaints involving pharmacy students and pharmacists. My focus is simple: fairness and impartiality. Every single decision I make will prioritise public safety, upholding high standards of practice, and ensuring that pharmacists feel supported, not sidelined. It’s about finding the balance and building a stronger profession, together.
