How the quality of a patient’s story can make or break their healing journey ( trade secrets after 7+ years as a medical social worker) Consistency is essential. If the provider says the patient needs SNF and PT says home, we have a problem. If the initial assessment says the patient has been non-verbal for 3 days but the family says 3 months, we have a problem. If the home health says the patient has severe dementia but the H&P says patient is alert and oriented, we have a problem. If the discharge plan is home, but the patient has no home, we have a problem. When we bring these to light in the patient’s story, we can better address their real needs. Admissions read the chart before they meet the patient.
