"The health system is one of the determinants of health. Moreover, there is considerable evidence that hospitals, and the health systems they are embedded within, contribute to health inequities. Whereas primary and community based health services are likely to have explicit goals of addressing health inequity and prioritising local community needs, hospitals have been slower to adapt and respond effectively to the circumstances of indigenous people and other marginalised population groups such as poor people. Overcoming the institutional inertia of hospitals is important because their power and status makes them uniquely positioned to influence the rest of the health system, including how money is distributed between prevention and primary and acute care." Nothing new here but there are excellent examples within this article that remind us all of the opportunities health systems have to eliminate health disparities. As I continue to examine the topic of health disparities, I came across this report that looks at those disparities in detail from an ethnic perspective. This is a topic that warrants not only discussion, but a change in policy to address these disparities. These are some key findings that stood out for me below: 1. Persistent Disparities: Significant health and healthcare disparities exist across racial and ethnic groups, with Black, Hispanic, and AIAN populations experiencing worse health outcomes compared to white populations. 2. COVID-19 Impact: The pandemic exacerbated these disparities, leading to greater declines in life expectancy and highlighting vulnerabilities in healthcare access and quality for these groups. 3. Regional Variations: Health outcomes and access to care vary widely between states and regions, with certain states showing higher rates of preventable mortality and worse healthcare performance for minority groups. 4. Insurance Coverage Gaps: Black, Hispanic, and AIAN people are more likely to be uninsured or underinsured, which contributes to delays in care, higher medical debt, and worse health outcomes. 5. Quality of Care: Racial and ethnic minorities often receive lower quality care, face discrimination in healthcare settings, and have limited access to primary and preventive care. 6. Policy Influence: Historical and ongoing policy decisions have led to economic suppression, unequal educational access, and housing segregation, all of which negatively impact health outcomes for people of color. 7. Call for Reform: The report calls for policymakers to address these disparities through comprehensive healthcare reform, including Medicaid expansion, improving access to quality care, and investing in social services to support marginalized communities. In the 2024 State Health Disparities Report from the The Commonwealth Fund, data shows there are still profound racial and ethnic disparities in health, well-being and life expectancy in the U.S. One key factor cited is unequal access to primary care. The Commonwealth Fund released a new report on April 18th which examines racial and ethnic disparities in healthcare access, quality, and outcomes across the U.S. The foundation collected data for 25 health system performance indicators, specifically focusing on health outcomes, access to health care, and quality and use of health care services for Black, white, Hispanic, American Indian/Alaska Native (AIAN), and Asian American, Native Hawaiian, and Pacific Islander (AANHPI) populations. The tool's goal is to guide policymakers, health system leaders, and community stakeholders to investigate the impact of current and past health policies on different racial and ethnic groups.
