Last week I had the chance to join an incredible recording session featuring a conversation between a patient and one of their consultants. They both spoke candidly about the importance of shared decision-making in healthcare appointments, especially for patients living with rare diseases. Hearing their stories shared, seeing the passion with which they spoke, and witnessing their commitment to advocating on behalf of their communities was incredible. Definitely one of those moments that really hits you. Shared decision making isn’t just about creating care that’s clinically smart; it’s also deeply personal and makes a meaningful difference to those individuals and their families we aim to support. What would make it most difficult for caregivers? Half enough primary care, mental health, geriatric, womens health, basic surgical workforce Practices with fewer team members and lesser team members also due to the financial design and including ever lower payments as levels of physicians get lower Team members more and more distracted by disruptions in the practice, the health care design, via micromanagement, and in the community Deficits of social supports, transportation Worst public plans Worst private employer based plans which arise from the weakest employers Concentrations of elderly, poor, disabled, and weakest employers that set deficits of workforce in concrete and result in the inability to recruit or retain the better employers with their better health plans Plans that add to medical debt and worsening patient finances Performance based/value based payments and ratings where the populations are inherently lower in outcomes and in dozens of drivers of outcomes - pay them less and penalize them more Better health insurance being designed away with government, health care, and education jobs cut or centralized away All of the above is present in the 2621 counties lowest in health care workforce that have never had much help except Hill Burton and the first decade of Medicare and Medicaid illustrating the need for additional funding and better support which has been denied by design for 41 years and still going.
