Scenario You are working as HSE Manager at Al Mahmoud Construction Company. An accident occurred when one your company workers fell and suffered a broken hand injury. The worker was using an unsafe scaffold while building the wall. The company did not provide a full-body harness to the worker, and the supervisor was far from the area and did not provide a safe system of work, in addition no ladder was installed, which forced the worker to climb the scaffold in an unsafe manner. The worker only received safety orientation training and did not receive work-at-height training. Additionally, the wind was strong and the weather was dusty. The scaffold was built on soft, sandy ground, was not equipped with a handrail, and was overloaded with bricks. Due to a lack of manpower, the worker was forced to work long hours after the official end of working hours and without breaks, leading to fatigue. The worker was further distracted due to psychological fatigue as the supervisor was bad-tempered and constantly threaten workers with dismissal. The company is old and does not actively seek modern technologies to meet health and safety legal requirements. The manager of Al Mahmoud company did not issue safety policy and No risk assessment was developed for working at height. After the worker fell from the scaffold, the scaffold fell onto the wall and damaged the new building. The workers did not report any of these unsafe acts and conditions to employer because they know that he do not care. Task 1 Explaining to the employer obligations to workers 1- What employer obligations are likely to have been contravened, in this scenario? Human Factors in Marine Safety Management: Higher performing organizations determine what "tasks" in key process's (i.e. Navigation, Cargo Handling, Maintenance, High Risk Ops.) are "Safety Critical". One model for determining safety criticality is to use specialize risk tools. Another model, that is simpler: 1. identify tasks 2. determine the significance of the hazard(s) 3. list the barriers & layers of protection 4. score the barriers effectiveness 5. brainstorm Performance Influencing Factors (PIFs) 6. score likelihood the task will fail 7. make criticality determination There are important steps to take, once a task is considered "Safety Critical". A very good example of self-awareness and self-control is when we are at a train station with many trains passing by. As we sit there, we do not immediately jump onto any train, do we?!! Similarly, why should we let ourselves plunge into any problem and challenges that come our way? A sense of direction and purpose helps us recognize the right train that will take us to our desired destination. Once we achieve this clarity, we can confidently catch the right train. This careful consideration and deliberate action are at the heart of self-awareness. So next time, pause and observe, identifying which train we want to board and go for it!
