A growing problem in OBGYN is the new recommendations (March 2023) regarding breast density and the need for whole breast ultrasound (if Class C or D). Recent studies have shown that 50% of breast cancers are missed with Class D breasts. The radiologist (my brother is one) will recommend breast MRI. It is too difficult for our office to precertify the MRI (process is cumbersome). The insurance companies should have an algorithm with the protocol established and covered with follow-up with a breast surgeon that is also covered automatically. Another issue is a patient getting annual mammograms for years (without a physician order) and then having a suspicious mass that needs a biopsy. She has not been seen in our office for many years and we get a call about this patient needing additional studies. Failure to diagnose breast cancer is a significant malpractice problem. The goal should be what is in the best interest of the patient so that she can get the care she needs with timely evaluation and follow-up.

Marketing a new diagnostic device is challenging for a disease as relentless as #endometriosis. New tests are met with skepticism. Early studies of the ENDOSURE TEST showed promising results, particularly in the United Kingdom. We spoke to Ian Fairfax, director at Lawmed, for his insights on reception of the TEST in the UK: “We initially took the ENDOSURE TEST to 2 centers – Birmingham and Worcester. Both are UK Endometriosis Centers. Patients at these Centers have known endometriosis. After seeing multiple doctors and undergoing diagnostic laparoscopy, they’re likely at stage III or IV. These Centers perform endometrial excision. There are about 85 of them in the UK. We began doing 4 patients in each center. Some interesting results became apparent immediately. A nurse who performs the test brought her daughter in, who previously had 'complete surgical removal' of endometriosis. Because she remained symptomatic, the surgeon brought her back for the ENDOSURE TEST. We performed the TEST; she was positive for endometriosis accounting for her pain. An ultrasound was performed to verify the ENDOSURE TEST result. Once confirmed, the surgeon was quite happy with the ENDOSURE TEST accuracy. The patient’s mother told Lawmed, ‘I want you to know my daughter’s ultrasound revealed an endometrioma. Thank you!’ The following day at the Worcester Endometriosis Center, after conducting several ENDOSURE TESTs, the endometriosis nurse came to me and said, ‘Ian, we have a staff member who had surgery here some time ago, and her pain has come back. She was told she had complete endometrial excision. Any chance we could perform the TEST on her?’ I said of course we can! We performed the TEST, and she was found positive for endometriosis. Understandingly, the surgeon explained how excision of endometrial lesions can be challenging because operating specialists can only excise what they are able to see during the procedure. He stated, ‘Some small endometrial lesions can be obscured anatomically during the surgery, particularly when working deep in the pelvic cavity. This can lead to hidden, left behind endometrial lesions which we know can grow and spread.’ He was most pleased the ENDOSURE TEST caught this, and he enthusiastically pointed out he now believes that this breakthrough technology can fill the current gap in effective low-cost, post treatment follow-up. 'Since there is no other TEST that does this, my experience helped me to realize how this highly accurate, 30-minute ENDOSURE TEST is uniquely positioned to allow routine post treatment monitoring, enabling me to track individual response to treatment.'
