A meta-analysis compares duodenum-preserving pancreatic head resection (DPPHR) with pancreatoduodenectomy (PD) in benign pancreatic tumors, suggesting DPPHR as a less risky and equally effective surgical option. With a 90.57% accuracy in preoperative benign tumor diagnosis, DPPHR shows lower in-hospital mortality and reoperation rates, and comparable tumor recurrence risk post-surgery. This evidence positions DPPHR as a potentially preferable approach for certain benign, premalignant cystic, and neuroendocrine neoplasms of the pancreas, balancing oncologic outcomes with patient safety. A #pancreaticoduodenectomy, also known as the "Whipple" is a complex surgical procedure most commonly known for resecting pancreatic tumors. If cancer is localized this procedure can extend life expectancy despite the overall poor prognosis of pancreatic cancer. It also can be performed for other conditions including pancreatitis, pancreatic neuroendocrine tumors, and other times of digestive cancers. During this procedure portions of the pancreas and small intestine (duodenum) will be removed along with parts of the gallbladder, bile duct, and stomach. Risks associated with this procedure include infection, anastomosis leak, diabetes, bowel changes, and difficulty digesting food. There is an old saying that goes, "if you torture the data long enough, it will confess to anything". Is this abstract, presented at ASCO GU 2024, one of those instances? A major question with adjuvant trials is whether we are actually curing more patients, or only delaying disease recurrence. In CheckMate 274, which looked at 1 year of adjuvant nivolumab vs placebo in muscle-invasive urothelial carcinoma after surgery, one of the concerns was that it only demonstrated a ~12-month improvement in disease-free survival (DFS), with no overall survival data published because the data was not mature. This abstract attempted to answer these concerns by estimating the cure rate using a mixture cure model, with cure defined as 5 years of DFS. The result isn't surprising (nivolumab cured more patients) because it was based off the DFS data. It's hard to see what audience this abstract was intended for, because clinicians who are waiting on the overall survival data wouldn't be convinced by these extrapolation methods. There are some genuine positives to the trial - the DFS curves remained separated with longer follow-up, and the PD-L1 ≥ 1% and neoadjuvant cisplatin subgroups showed much better DFS improvement. Nivolumab may eventually demonstrate overall survival, especially in the subgroups with very significant DFS benefit, but it's better to wait for the results to mature rather than extrapolating them. #opdivo #bladdercancer https://lnkd.in/eNtuz9p2
