Advancing medical standards of care demands immense dedication and expertise, and I am privileged to work with many committed professionals and clinicians who are enhancing our understanding of atrial fibrillation (Afib). As we kick off #AfibAwarenessMonth, it’s also fitting to highlight the release of the newest 2024 European Society of Cardiology AF Guidelines, developed in collaboration with EACTS. Afib affects 37 million people globally, and reducing this number requires a collaborative commitment to evidence-based practice and improving outcomes globally.
The key updates in these guidelines include:
Concomitant Surgical AF Ablation is now rated Class I.
Surgical LAA Exclusion upgraded to Class I.
Hybrid AF Ablation is classified as Class IIa.

Patients with unexplained embolic strokes might have a right-to-left shunt (RLS) due to a patent foramen ovale (PFO). Studies show that closing a PFO can reduce stroke risk in select individuals. Sharing a presentation I made earlier in June to the Foundation for Healthcare Quality Care Outcomes Assessment Program annual meeting. I presented the findings from a quality improvement program I ran to discover the impact of peer coaching for cardiac surgery patients on anxiety and depression and rates of cardiac rehab participation.
The results were clear - patients benefitted from connecting with other patients. The quality improvement study led to the creation of PatientCircle (https://lnkd.in/g9htyA2N) where we're building an intelligent peer community platform for surgery patients.
Here's some information that you may not know about the human and healthcare costs of surgery:
A substantial proportion of cardiac surgery patients - ranging from about 20% to up to 50% - experience depression either before or after their procedure. This is significantly higher than depression rates in the general population, which are estimated at 5-9%
A study of Medicare beneficiaries with cancer found that patients diagnosed with anxiety and depression after their cancer diagnosis incurred an additional $17,496 in healthcare costs annually compared to cancer patients without anxiety/depression. This represents an 80% increase in the total cost of cancer care per year
Post-surgery depression increases mortality and morbidity, health care costs, and re-hospitalizations, reduces quality of life and vitality, and delays return to work
For patients undergoing rotator cuff repair surgery, those with comorbid anxiety or depression had 7.05% higher healthcare costs in the year following surgery compared to those without mood disorders. This translates to an estimated $62.3 million in additional annual costs across all rotator cuff surgeries performed in the United States
Increasing cardiac rehabilitation participation from 20% to 70% would save 25,000 lives and prevent 180,000 hospitalizations annually in the United States
If you or a friend or family member has had or will soon have an experience with heart surgery or any surgery, the presentation is worth your time. Most patients don't connect with other patients who can relate, and from our study, we can see that it's a good idea.

Armin Garmany, and Drs. Sara Inglis, Atta Behfar, and Andrew Rosenbaum at the Mayo Clinic published a nice case report demonstrating the usefulness of #PVloop monitoring during #LVAD speed optimization.
#Hemodynamics revealed a clear trend toward worsening right ventricular systolic function with increased LVAD speed, despite beneficial left ventricular unloading. As a result, the medical team advised adding inotropic therapy to support the right ventricle while increasing LVAD speed.

Improving patient-physician communication through shared decision-making is essential for enhancing outcomes and satisfaction in healthcare. A recent study focusing on patients undergoing Hallux Valgus correction found that perception of SDM was related to demographic and clinical factors. Patients with symptoms of depression and lower socioeconomic status reported lower levels of SDM. However, interestingly, postoperative outcomes were not directly influenced by SDM scores.
