I want to share some advice for psychiatric nurse practitioners Administrative responsibilities can take up a significant amount of additional time if time is not allotted specifically for these tasks. As a psychiatric nurse practitioner, you are responsible for mental health treatment of patients who have appointments with you, but there are also patient needs that go beyond their appointment time. These “admin time” tasks include callbacks to family members, pharmacies, doctors and/or therapists, reviewing lab results, completing prior authorizations and fulfilling paperwork requests. Before you accept your first job or a new job, it is important to inquire about dedicated admin time and/or assess how you will dedicate time to these tasks and if that is feasible given the patient case load. In my last employed role (32 hours per week), 4-6 of those hours per week were dedicated to admin time.

The effects of betrayal on nurses can be profound, leading to emotional exhaustion, burnout, moral distress, a sense of isolation, decreased job satisfaction, a loss of trust in colleagues or the healthcare system, and even consider leaving the profession. This emotional toll can also impact their mental health, self-esteem, and overall well-being. To address this, organisations should focus open communication (open dialogue and feedback channels), strong ethical standards (accountability structures), accessible support systems, leadership training, and fostering teamwork. These can help rebuild trust, reduce stress, and create a healthier work environment for nurses.

How I Came to Understand the Deep Impact of Betrayal in Nursing Betrayal in the workplace is more than just a “bad day” or a moment of disappointment—it’s a profound break in trust that can change the way we view our profession, our colleagues, and even ourselves. As nurses, we enter the field driven by compassion and commitment, believing our organizations will support us, protect our well-being, and stand by ethical values. But what happens when the very institutions we trust fail us? Institutional betrayal in healthcare is real, and it has lasting consequences. When we’re faced with a lack of support, discrimination, or punitive measures instead of guidance, it feels like a breach of the very trust we rely on to perform our demanding roles. This betrayal doesn’t just affect morale—it impacts mental health, motivation, and even patient care. In a profession where we’re taught to prioritize the needs of others, experiencing this kind of betrayal can leave us questioning our place and our purpose. I learned this the hard way. Like many, I’ve faced moments where I felt abandoned by the system, left without the support needed to succeed or even just to cope. And the psychological impact of that betrayal was deeper than I initially understood. It made me wonder: if the foundation of trust isn’t there, how are we supposed to build a sustainable, compassionate workforce? To truly support our nurses, healthcare institutions need to recognize the impact of these betrayals. We can’t keep telling our nurses to “be resilient” without addressing the root of the issue: the need for a culture of trust, empathy, and fairness.
