A #pancreaticoduodenectomy, also known as the "Whipple" is a complex surgical procedure most commonly known for resecting pancreatic tumors. If cancer is localized this procedure can extend life expectancy despite the overall poor prognosis of pancreatic cancer. It also can be performed for other conditions including pancreatitis, pancreatic neuroendocrine tumors, and other times of digestive cancers. During this procedure portions of the pancreas and small intestine (duodenum) will be removed along with parts of the gallbladder, bile duct, and stomach. Risks associated with this procedure include infection, anastomosis leak, diabetes, bowel changes, and difficulty digesting food. **Understanding Parathyroid Adenoma and Its Hidden Dangers** A 65-year-old woman diagnosed with left breast cancer (cT2N0M0) underwent a preoperative CT scan, which revealed a suspicious lesion near her thyroid. Further tests showed elevated calcium levels (13.7 mg/dL) and a high parathyroid hormone (PTH) level of 560 pg/mL, leading to a diagnosis of parathyroid adenoma – a benign tumor causing primary hyperparathyroidism. Even though she was asymptomatic, this condition can lead to serious complications like kidney stones, bone fractures, and pancreatitis. In fact, her CT scan revealed asymptomatic bilateral renal stones. She successfully underwent surgery to remove the parathyroid adenoma during her breast cancer surgery. Post-op, her PTH levels dropped to normal (11.2 pg/mL), and she is now recovering well. This case underscores the importance of early detection and treatment of parathyroid adenoma to prevent the silent but serious complications of hyperparathyroidism. **Key Points to Remember:** - Elevated calcium and PTH levels can indicate parathyroid adenoma. - Timely surgery can prevent complications like kidney stones and bone damage. - Even asymptomatic patients are at risk for long-term complications. Circulating large extracellular vesicles as diagnostic biomarkers of indeterminate thyroid nodules: multi-platform omics analysis ➡️https://lnkd.in/eCKVWp_i This is a proof-of-concept study which demonstrates for the first time the potential of circulating extracellular vesicles to discriminate between non-cancer and cancerous indeterminate thyroid nodules. Mir-195-3p was upregulated, while five other microRNAs were downregulated (mir-3176, mir-205-5p, mir-3529-3p, let-7i-3p and novel-hsa-mir-208-3p) in patients with cancer compared to non-cancerous nodules. Furthermore, three large extracellular vesicle proteins were significantly upregulated (kallikrein-related peptidase11, alpha-1-acid glycoprotein 2 and small integral membrane protein 1), while 20 proteins were significantly downregulated. The top two downregulated proteins were chemokine (C-X-C motif) ligand 7 and tubulin β-1 chain. Colorectal cancer prevalence in faecal immunochemical test non-returners: potential for health inequality in symptomatic referral pathways ➡️https://lnkd.in/dYv5FHCG Faecal immunochemical testing has been adopted into clinical pathways as the first-line test to investigate people with symptoms suspicious of bowel cancer. Those who do not return faecal immunochemical tests are not well studied.
