🌟 It was an exciting weekend for spreading awareness about painful diabetic peripheral neuropathy (DPN)! 🌟 Thanks to Drs. Dr. Jason Snyder and Michael A. Fishman for educating the American Podiatric Medical Association (APMA) podiatrists on how spinal cord stimulation (SCS) is a long-lasting treatment for patients with painful DPN. Our Pain Interventions team was also honored to attend the Annual Conference of the Association of Diabetes Care & Education Specialists (ADCES). We had the opportunity to meet with diabetes specialists and discuss how collaborating with pain managers on SCS therapy can be a game changer for their patients dealing with painful DPN. We are overwhelmed with how many podiatrists and diabetes specialists were excited to hear that there is an effective, non-drug therapy option for their patients with painful DPN. They shared how there are few effective pain relief options for their painful DPN patients and shared their interest to partner with pain physicians. I would like to share an interesting case from my clinic. The 28-year-old patient was referred to me with a complaint of sudden visual loss in the right eye after heavy exercise at the gym. He has no past medical history and no history of drug use. Upon examination, the intraocular pressure (IOP) in both eyes was 23 mmHg. Pigmentary dispersion syndrome (PDS) was evident in both eyes. During fundoscopy, the cup-to-disc ratio was normal, but evidence of central retinal vein occlusion (CRVO) was present in the right eye. Considering the risk factors for thromboembolic events, necessary investigations were performed. All exams were normal except for the borderline level of homocysteine. Given the patient's young age for CRVO occurrence, the presence of PDS in both eyes, and the borderline homocysteine level, what is the most important risk factor for CRVO occurrence in this patient? Can intense physical activity and a sudden increase in IOP be considered as main suspects? Should we limit intense physical activity in patients with pigmentary dispersion syndrome?
