We at Careteam Technologies are thrilled to be collaborating with you on this important initiative! Too many digital solutions don't do the necessary validation with researchers what patients, caregivers and providers really want and need, so it's exciting to have world-class leading expertise from Mount Sinai Hospital (Toronto), Sinai Health’s Drs. Carolyn Steele Gray and Michelle Nelson, together with Dr. Terence Tang at Trillium Health Partners to help create better digital tools for the future of healthcare - integrated care delivery. “Patients with complex health-care needs are at their most vulnerable when transitioning out of the hospital,” said Dr. Steele Gray. “Our goal is to ensure continuity of care, empowering patients and their families while equipping health-care providers with the tools to offer seamless support.” Technology can deliver that, but the right technology, workflow and user experience set up can make this a better process that avoids effort, frustration and burnout and actually brings in trust, reassurance and even joy! 💡New research suggests technology-enabled care at home is no riskier than hospital care 💡 A recent article in BMC Medicine suggests that in-patient level care delivered virtually or at home may be just as effective as traditional hospital care. The study looked at technology-enabled care models and found no evidence of an increased risk of readmission or mortality for patients receiving care at home. In-patient care at home, also known as virtual wards or hospital at home, uses technology to deliver care to patients in their homes. This can include remote monitoring, video consultations, and home visits from healthcare professionals. The study suggests that technology-enabled care at home is a promising option for patients who would otherwise need to be hospitalized. More research is needed to determine the long-term effects of this type of care, but the initial findings are encouraging as well as the cost implications associated with such innovation #healthcare #technology #homecare https://lnkd.in/eZU5UfNB thanks for sharing Dr. Joy Furnival CEng MIET https://lnkd.in/e3r58q5a Nick Prentice Nicola Medici Jenny Panes Francesca Wilson Jo Berry

At Telescope Health, we understand the challenges ACO providers face in managing both chronic care and annual wellness visits. Our partnership with Health Endeavors is designed to support providers by filling the gaps in patient care without disrupting established relationships. By engaging in Telescope Health's Provider Enablement Solutions, we help manage annual wellness visits for patients who might otherwise go unseen—ensuring that vital health information is funneled back to primary care physicians. This collaborative approach enhances patient outcomes while allowing providers to focus on critical care. Watch this clip to hear Matthew Thompson, MD, FACEP speaking on Empowering ACO Providers Through Collaboration in a discussion at the Health Endeavors' Innovators Summit. Research consistently demonstrates that patient engagement is vital for enhancing #healthcare outcomes. A recent, notable example of this is the bold initiative launched by Houston Methodist which implemented a post-discharge texting pilot program to a patient population of ~80K that significantly reduced the use of acute care resources and improved the patient experience. In this study, Houston Methodist demonstrated patients engaged via text experienced: ✅ 29% fewer readmissions & 23% less likely to be readmitted (w/in 30 days) ✅ 20% fewer revisits ✅ Significantly higher HCAHPS scores across all metrics Click through to see more detail on their findings ➡️ And kudos to our customer, Houston Methodist, for conducting such an outstanding study—featured in the BMJ! We’re proud to partner with such an innovator in this space. Read the full study » https://lnkd.in/gKdCW5Mh #patientexperience #healthtech #digitalhealth #healthit

Strongly encourage CA providers and stakeholders to listen in on this webinar. There are many important lessons we have from the past several years on what works and what is left to do in order to transform health care delivery to a scalable, sustainable model that disrupts health care to drive better outcomes including chronic care management and improved access. “Telehealth in California has expanded significantly to become part of health care’s new normal.”
I commend everyone working to advance telehealth policy and practice in California because you have made great progress in helping patients access care through telehealth. California has been a leader in telehealth policy – with many of the policies developed before the pandemic laying the foundation to make telehealth widely available when it was most needed. While we should celebrate this, more can be done to ensure health equity. We spoke with more than two dozen experts to get their advice on telehealth’s future. Join us on February 6 to discuss the roadmap to get us to #healthequity.
